Here is the e-mail conversation I had with Ed previously in reference to this site. The site was closed out last year.

Ed,
I appreciate the information. I will request that he submit an official letter stating his intent to not renew the permit. I
will also copy you on the inspection report.
Thanks,
C.T.

Thanks for the reminder. The permit has expired, but if we could ask Mr Meyer to send a letter indicating that they are
not renewing the permit, we can send a letter and close the site. He may have just gotten an invoice for yearly fees, so
this is a good time to finish it.

Subject: 3418TP - Myers Treatment and Processing Facility
Ed,
I recently inspected the subject site which is a stand-alone treatment and processing facility. I checked with Tony
Gallagher to see if he would be the contact for permitting questions, but he stated that the facility was not on his
database, so he felt that this site may still be under your direction as you signed the last Permit to Construct/Operate
for the facility back in 2010. Mr. Herb Myers recently passed away, and his son, Marty expressed that he has no plans to
renew the permit for the facility. Inspection found that all waste had been removed from the facility with the exception
of a very small amount of brush. The site has been graded to not impound water. So, my question is do we need an
3
official letter from the permit holder officially stating that they wish to close the site out? If so, who does this letter need
to be sent to? The permit for the facility has expired. I have attached a photo of the facility for your review.
Thanks,
C.T.
