The Permit to Construct and Operate will expire on September 6, 2027. This permit is the combination of two former notified LCID landfills, which were less than 2 acres each. The northern LCID began operating in 2006 with the notification number of N0712.The southern LCID began operating in 2018 with the notification number ofN1096. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None Pagel of 3 NORTH CARnLINAD_E Q�� OepaNnent W Environnnnbl puel� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. Signage has been placed at the entrance to the facility. Additional signage has been placed throughout the facility to aid in the operation of the LCID. 2. A gate has been installed at the entrance to the facility. 3. The sediment basin appeared to be functioning properly. Four fabric baffles run across the sediment basin. No overgrown vegetation was observed on the side slopes of the sediment basin. Continue to monitor and maintain the basin as required by the stormwater permit. 4. The access road to the facility is being managed properly at this time. An access road has also been established around the entire LCID to provide access for fire -fighting vehicles. Mr. Collie has placed concrete barricades around the access road to help prohibit unauthorized access to the site. Continue to maintain the access roadway to ensure access around the entire facility. 5. The side slopes have been reworked and slightly sloped since the last inspection. Mr. Collie stated that he is in the process of training a new operator to the proper compacting, sloping, and covering of the LCID. Continue to compact, slope, and cover the LCID waste to meet the three horizontal to one vertical slope ratio. 6. At the time of this inspection, the working face was less than the one -acre size limit. Continue to cover the waste with a minimum of six inches of soil cover monthly or when the working face reaches one acre in size, whichever occurs first. 7. No areas of exposed waste were identified in the southern portion of the LCID (former N1096). Leaf mulch has been placed on the side slopes of the southern portion of the LCID to help prevent erosion and to assist in the growing of vegetation. 8. The active northern portion of the LCID has been mown since the last inspection. Continue to mow and maintain the side slopes to allow for proper inspection. 9. Edge of waste markers have been installed around the LCID and can be seen from one to the next. Continue to maintain the edge of waste markers to ensure that they are visible around the LCID. 10. Stockpiles of broken concrete were observed at the facility. Mr. Collie stated that the concrete is scheduled to be crushed in the next month. He also stated that he has stopped accepting large amounts of concrete due to the large amount stored at the facility. Any rebar or metal would be removed prior to crushing and recycled. 11.
