Facility 10900 Pine Grove Ave., Mint Hill, NC 28227 LCID Permit No. 60AR-LCID Dear Ms. Moutos, On behalf of Tommy Price, Inc. ("TPI"), I write to provide the Solid Waste Section for the North Carolina Department of Environmental Quality, Division of Waste Management (the "Section") with the following requested documentation to initiate the process for change in business structure for TPI: REQUESTED INFORMATION As the current owners of the Tommy Price LCID facility, the following documentation would be required to start the process for change in ownership. A description of the proposed ownership change including affected facilities and associated permit numbers, the schedule for the proposed change in ownership or corporate structure, and contact name and information for the proposed or new owner or operator. (Once the Section receives the proposed new owner contact information, we can send a formal compliance history review questionnaire to Mr. Radilc This would include Mr. Radik's company name, address, email and phone) Ms. Sarah M. Box 847, Locust, NC 28097 ("Facility") Physical Address - 10900 Pine Grove Ave., Mint Hill, NC 28227 Permit Number - Land Clearing and Insert Debris (LCID) Permit No. 60AR-LCID Current Owner of Facility -Tommy Price, Inc. Corporate Officers • Paul Price, President; 704424-3747; paulfprice@bellsouth.net • Jeffrey Price, Vice -President; 704-548-9292; hgovou2006na aol.com • Mary Ann Price Conard; Vice -President; top=priceincaatt.net Description of Ownership Change The current owners, officers and shareholders of Tommy Price, Inc. ("TPI) propose to transfer ownership for Tommy Price, Inc. to Big P LLC (Buyer), a North Carolina limited liability corporation. Big P, LLC's principal office is located at 4817 East Fork Lane, Monroe. North Carolina 28110, with Managing Member Ruvim (Reuben) Radik and Executive Officer Ruslan Radik. Under the terms of the contemplated sale between Seller and Buyer, Seller would transfer certain TPI assets, including the LCID landfill associated with Permit No. 60AR-LCID referenced above, to Buyer and Buyer would assume all TPI liabilities, including without limitation all liabilities and regulatory requirements associated with the LCID referenced above. As part of the transaction, all current corporate officers of TPI would be vacated and replaced by officers selected by Buyer. Schedule for Proposed Changed TPI would like to proceed with the sale to Buyer as soon as possible, but no later than April 309 2024. In accordance with LCID Permit No. 60AR-LCID, Attachment I, Part IV., ¶8 and NCGS 130A-295.2(g), TPI requests that you accept this notice on behalf of the Section as satisfaction of the required notice of thirty (30) days prior to the change in business structure described above.
