ADDITIONAL COMMENTS:
1. Permit expired on May 8, 2022. Mr. Swaringen stated he believes the site stopped receiving waste in June
2022.
2. During July 26, 2022, inspection, Ryan Smith reviewed the requirements for closure with Mr. and Mrs.
Swaringen. Mr. Smith sent an email on October 18,2022 requesting written notice of intent to close be sent
to Claire Osborn Environmental Engineer with NCDEQ- solid waste section (Osborn@ncdenr.gov). To date
no notification has been received. Mr. and Mrs. Swaringen were informed on they need to send a letter to
Claire Osborn.
3. Lakeview has until June 9, 2023, to submit their intent to close to NCDEQ- Solid Waste Section to prevent
compliance actions.
4. Vegetation is well established; no exposed waste was observed.
5. Some standing water was observed during the inspection due to rainfall prior to inspection.
6. Facility Annual Report for FY 21/22 was received by the Solid Waste Section on 8/18/22.
