PERMIT APPLICATION CHECKLIST
SEPTAGE MANAGEMENT FIRM (RESTAURANT Only)
NC Department of Environmental Quality
Division of Waste Management- Solid Waste Section
❑ Complete all questions on both pages of the application, including:
o Contact information, including email
o Estimate of gallons pumped within the calendar year
❑ Complete and submit the Restaurant Certification Statement form.
❑ Enclose a completed Procedures to Clean a Point -Of -Use Grease Interceptor or a signed
copy of your own restaurant cleaning procedures.
❑ Make check payable to: DIVISION OF WASTE MANAGEMENT
o Restaurant fee - $500
❑ To pay online with a credit card, debit card, or checking account, you will need the
invoice for instructions.
❑ Mail payment (unless you are paying online) and all application materials to:
