LOS Issued on January 22, 2020 and expires on June 14, 2054. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS: 01 - This facility has required signage and has locking gates at the entrance. 02 - The access roads are of all-weather construction. 03 - Waste is being transported to WM-Atlantic Waste Disposal Landfill in Waverly Va. or transported to East Carolina Regional Landfill (permit# 08-03) in Aulander, Bertie Co. 04 - The tipping floor and interior wall and fans were clean during this inspection. 05 - Leachate drains were clean during this inspection. 06 - Facility has 3 Leachate tanks. Last dated removal record was dated 03/30/2023. 06 - No Windblown trash was noted at this facility during this inspection. 07 - Screening records were available and reviewed. NO DOCUMENTED SCREENING RECORDS SINCE 08/05/2023. 08 - The trailer loading bay was clean during operating hours. 09 - Scrap metal is pulled from the waste stream at this facility and is placed into containers for recycling. 10 - Operating hours for this facility are: Monday – Friday, 8am to 4:30pm and Saturday, 8am to 12pm. 11 - The LOS Permit to Operate was issued 1/22/2020 and expires 6/14/2054. 12 - Permit and operations plan were available and reviewed. 13 - Scale tickets were not reviewed. 14 - Facility Certified Transfer Station Operations Specialist and 1 MOLO: Janice V. Brown – expires October 19, 2025 Ricky Holley – expires August 28, 2026 Carl Swain, expires – November 03, 2024 Clinton Mercer, expires October 19, 2026 Troy Ward – expires, February 02, 2026 Brad Gardner, MOLO, expires September 15, 2023 15 -Training records were reviewed. Need to discuss Permit and Operations plan during the required 8 hours of training that is required annually by Rule: 15a ncac 13b .0405 (7). Document attendees and date and times. 16 – Most trailers from past inspections have been repaired. Some unloaded trailers (306, 309 and 310) are in need of repairs to rear door seals and latches to prevent waste and leachate from escaping from the trailers. 17 - Facility staff /management needs to monitor the trailers more often. 18 - No leachate leaks found on loaded trailers. 19 - Digital pictures taken during this inspection with several attached to this report. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
