Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. Proper signage was observed at the entrance of the facility. 2. The facility is permitted to receive solid waste generated within Iredell County. 3. The facility accepts an average 889.16 tons per day. Operations Inspection of the MSW Landfill Unit: 4. The facility was currently working in Phase 6. 5. The landfill was actively receiving waste during the inspection. and set aside to be properly disposed at the tire collection area. 6. Edge of liner markers were observed. 7. All access roads were in good condition. 8. Windblown waste was being collected during the inspection. 9. The working face was small and compact. 10. No unacceptable waste streams were observed in the MSWLF. Tires had been separated out of the working face Operations inspection of the White Goods Collection Area: 14. This area was well managed at the time of inspection. 15. White goods containing Freon are separated from other metals and stored in an upright manner. 16. Facility staff extract the Freon from all white goods. Operations inspection of the Electronic and Paint Collection Area: 17. This area was well managed at the time of the inspection. 18. Electronics were palletized and staged for Powerhouse Recycling in Salisbury to pick-up. 19. Latex paint collected in five (5) gallon containers is mixed with the Posi-Shell. Operations inspection of the HHW Area: 20. The facilities Household Hazardous Waste Generator Identification Number is NCPH0492001. 21. The Household Hazardous Waste Facility consists of a permanent containment unit that has three separate lockers for temporary storage of household hazardous waste (Locker #1 — Flammable liquids; Locker #2 — Fluorescent Bulbs; Locker #3 — Pesticides). All three lockers were secured and locked. 22. The floor grates and space beneath the grates were clean and free of debris. 23. Each of the three lockers in the containment unit are equipped with an Ansul Fire Suppression system. 24. The facility offers an HHW event once a month to the residents of Iredell County which is held the last Saturday of each month. 25. The county has a contact with ECOFLO, Inc. to collect the household hazardous waste. Operations inspection of the Tire Collection Area: 26. Tires are stored on the ground. When enough tires have accumulated for a load landfill staff contact New River to deliver a trailer. 27. The facility received 1,880.85 tons from 1/1/2022 to 12/31/2022 and have received 1,174.01from l/1/2023 to 7/19/2023. 28. New River collected 1,922.78 tons from 1/1/2022 to 12/31/2022 and have collected 1,167.00 from 1/l/2023 to 7/19/2023. Page 2 of 3 FACILITY COMPLIANCE INSPECTION REPORT DE QI Division of Waste Management Solid Waste Section Operations inspection of Mulching and Grinding/Yard Waste/Type 1 Small Composting Area: 29. This area was well managed at the time of the inspection. 30. Land clearing waste and pallets are kept separated from the yard waste that is required to be composted. 31. Farmers Mulch & Rock was on -site grinding during the inspection. 32. Painted pallets are removed to be used in the electronic collection area. Operations inspection of the Asphalt ShingleRecyclin Area: 33. The asphalt shingle recycling area was well managed at the time of the inspection. 34. Bunker A was closed (left side) and has been tested and 516.31 tons are ready to be hauled. Bunker B (Right side) is open and has 22.70 tons. 35. Asphalt shingles are collected by Maymead Asphalt for recycling. Records Review 36. A copy of the current permit and operations plan are maintained in the landfill office. 37. Reviewed semi-annual groundwater, surface water and leachate monitoring reports (February 2022 to March 2023). 38. Reviewed leachate discharge records for 2022 and 2023. Leachate lines are flushed once per year and videoed every five years. Leachate lines are scheduled to be flushed and videoed August 21, 2023. 39. Continue to work with the Section on the approved Remediation Plan for MP-6A monitoring well. 40. Random inspection reports were reviewed from June 29, 2022 to June 28, 2023. Facility staff perform 2-3 random inspection per week. 41. Yard waste compost temperature records were reviewed and are being maintained in accordance with the rules. 42.
