A copy of the form is required to be sent to the County Manager of each county from which your facility received waste. 4. A copy of the report will automatically be forwarded to your inspector when the form is submitted. 5. The person submitting the form will receive an email confirming the report is accepted when Solid Waste staff complete their review.
