PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. Date of Last Inspection: NA Long.:-80.2135441 The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility is a Yard Waste Notification that accepts yard waste. The facility is not open to the public. 2. No composting occurs at the facility. 3. Facility access is secured by a locking gate with the site secured by a perimeter chain link fence. 4. Facility roadways were of all-weather construction and in good condition at the time of the inspection. 5. The facility appeared to meet all Yard Waste Notification buffer facility requirements. 6. The facility operating area is located just south of the facility entrance. Mr. Hendrick stated that the rest of the site is used for equipment storage. 7. Processed mulch was observed to be stockpiled along the roadway just south of the entrance. The window appeared to be of a manageable size with an adequate buffer to allow access in case of fire. 8. The yard waste stockpile was inspected with no unacceptable material found. The stockpile is located near the fill area for the site. Mr. Hendrick stated that the yard waste material on site is brough from his business only. 9. Processed materials on site did not exceed the 6,000 cubic yard maximum. 10. Mr. Hendrick stated that the process material is used as an erosion control measure on his sites. 11. Ensure that the facility has proper signage as specified in 15A NCAC 13B .1406(9). 12.
