PERMIT # 3004-Compost-2014 DATE AND TIME OF FIRE: 03/30/2023 HOW WAS THE FIRE REPORTED AND BY WHOM: P, 2:30 am Fire was reported to local Fire Department by traveler on I-40, Fire Department called Wallace Farm supervisors immediately. LIST ACTIONS TAKEN: Heavy equipment was mobilized to pull out mulch that was burning and separate it from non -burning material. WHAT WAS THE CAUSE OF THE FIRE: Spontaneous combustion is only factor that could be determined. DESCRIBE AREA, TYPE, AND AMOUNT OF WASTE INVOLVED: i'he fire started in the most recently created mulch windrow next to the new shop and debris pile. Approximately 100 cubic yards of material was burned. WHAT COULD HAVE BEEN DONE 1'0 PREVENT" "1"H15 HKE: Limit anount of mulch allowed to come into this area at this facility. Monitor this area more frequently. DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS: Reduce the amount of mulch coming into this area. Increase the ferquency of fire watch on this windrow.
