Reynolds 7612 Pearman Quarry Road Kernersville, NC 27284 PARTICIPANTS: Chuck Kirchner, Environmental Senior Specialist – Solid Waste Section (SWS) Johnny Reynolds, Owner/Operator – 7716 PQR LCID STATUS OF PERMIT: Permit To Operate (PTO) issued August 3, 2021 Permit To Construct (PTC) issued in conjunction with PTO PTO expiration date August 3, 2026 PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS On March 30, 2023, Chuck Kirchner met with Johnny Reynolds to conduct a Phase 2 pre-operational comprehensive inspection of the 7716 PQR LCID Landfill. The LCID landfill is located at 7700 Pearman Quarry Road in Kernersville, NC in Guilford County. 1. The facility is a land clearing and inert debris (LCID) landfill owned and operated by Johnny Reynolds. 2. The facility is open to the public. 3. The facility is in operation Monday through Saturday 8:00 am to 5:00 pm or by appointment. 4. The facility is permitted to receive waste generated within Guilford, Randolph, Davidson, Forsyth, Stokes, Caswell, and Rockingham Counties. 5. The facility is permitted to receive land clearing waste, yard trash, untreated and unpainted wood, uncontaminated soil, inert debris, and (used) asphalt. 6. Landfill is currently placing LCID material in Phase1. 7. Working face appeared small and compact. 8. A stockpile of soil is located near the entrance which will be used for cover material according to Mr. Reynolds. 9. A small amount of incidental waste pulled from the working face is kept in a central location. Mr. Reynolds stated that this waste is picked up periodically and properly disposed of. 10. No unauthorized waste material was observed in the working face of the landfill. 11. Per 15A NCAC 13B .0566(3), “Solid waste shall be restricted to the smallest area feasible and compacted as densely as practical into cells.” 12. Per 15A NCAC 13B .0566(4), “Adequate soil cover shall be applied monthly, or when the active area reaches one acre in size, whichever occurs first.” 13. Adequate soils for cover shall be maintained on site as discussed in the field. 14. Excavation, grading, and fill material side slopes shall not exceed three to one (3:1). 15. Silt fence posts painted red are used as permanent edge of waste markers. 16. On March 29, 2023 Mr. Reynolds submitted a written certification prepared by Piedmont Environmental Associates, P.A. to certify that the elevations of the landfill base have been constructed in accordance with those shown in the approved Site Plan (FID 1587016). 17. Fill has been placed in Phase 2 area and appears to meet Section B-B profile standard on approved plan sheet stamped 6/22/21 (FID 1587016). View of the Phase 2 fill area. The facility annual report (FAR) was submitted to the Solid Waste Section on July 31, 2022. From July 1, 2021 through June 30, 2022 the facility received an estimated 1,575 tons of LCID material. 19. Facility is operating under a Guilford County Sediment and Erosion Control Plan, number 21-03-STOP-02900, issued March 18, 2021. 20. Erosion control measures are installed well and appear to be functional. 21. Some logs are kept to the side and are donated as firewood to the elderly according to Mr. Reynolds. 22. No open burning of solid waste is permitted on site. 23. The owner or operator shall provide verbal notification to the Division of fires that occur at the site within 24 hours of the fire and shall submit written ‘Fire Notification Form’ to the Division within 15 days of the fire. 24. Access roads and lay down area are of all-weather construction. 25. The facility is secured by a locked gate. 26. The facility has a proper sign. View of the PQR LCID sign.
