LOS Permit issued 1/7/2019 for phases 1-6a. Expires 4/4/2056. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS: 01 - Signs posted at the entrance have all required information as stated in the permit and this facility is adequately secured by fencing and locking gates. 02 - Interior roadway is of all-weather construction and maintained in good condition. 03 - This facility has several Certified Landfill Operations Specialist: Partial listings of operators: Carl Swain, expires 11/3/24, Warren Stallings, expires 11/20/23, Lisa Key, expires 5/3/24and Janice Brown, expires 4/26/25. Several other operators are available but not listed on this report. 04 - This facility also has a Certified Manager of Landfill Operations. Brad Gardner, Solid Waste Director, MOLO, expires on 09/15/2023. 05 - Edge of waste markers are present. 06 - 2 Leachate breakouts were noted and are in need of repair before the leachate leaves the landfill footprint. 07 - Cell 6B, Cell #1 was approved to operate on 10/22/21. Currently open and the working face. Better cover is needed. 08 - The working face is kept in a small area, less than a 1 half acre. 09 - There was a small amount of windblown trash in need of picking up by the end of the working day on and off of the working cell. 10 - LC area looks good during this inspection but full. Grinding contractor to set up next week. 11 - Concrete area was kept in good order. Move pile of comingled Inert Debris to C&DLF. 12 - Permit and Operations plan were reviewed during this inspection. 13 - Cover records were reviewed during this inspection. Need to add more cover. Facility is mixing 50/50 Soil/Mulch (SM) alternative daily cover at the barrow pit for transportation to the working face. 14 - Incoming load tickets were not reviewed during this inspection. 15 - Screening records were not reviewed during this partial inspection 16 - Scrap tires are loaded into an open top box trailer. Area looks ok. 17 - Semi Annual Monitoring records were available and reviewed. Samples were taken on 1/25/22 and 8/31/22. 18 - Due to the cover and leachate breakout issues, this facility needs to monitor the site daily (multiple times) for these issues. Perhaps the facility needs to come up with a daily inspection check off form to ensure these issues are caught and corrected in a timely manner. 19 - Yard waste area appears to be in good shape. 20 - Facility staff needs to come up with a plan to slow down the erosion of the West sideslope. Recent rainfall has caused bad rills on the West sideslope. 21 - Digital pictures were taken during this inspection with several attached to this report. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
