STATUS OF PERMIT:
Closed. Site closed in accordance with the Closure letter dated December 29, 1995, and NC Solid Waste Management
Rule 15A NCAC 13B .0510.
PURPOSE OF SITE VISIT:
Comprehensive Facility Inspection.
STATUS OF PAST NOTED VIOLATIONS:
1. 15A NCAC 13B .0502(2)(a)(ii) — CORRECTIVE MEASURES ONGOING for landfill gas (LFG)
exceedances at the facility boundary.
As required, the facility has continued to submit monthly Operations, Monitoring, and Maintenance Reports to the
Section for the Landfill Gas Collection and Control System (LFGCCS) and the facility perimeter LFG monitoring wells.
Please continue to coordinate with Section Hydrogeologist, Jaclynne Drummond, at email
jaclynne.drummond@ncdenr.gov, for ongoing facility LFG corrective action measures and submittals.
(Note: the 15A NCAC 13B .0500 Rules were readopted effective January 1, 2021. Post -Closure Care Requirements for
closed MSWLF Units exempted from other MSWLF Rules are currently in Rule 15A NCAC 13B .0510.)
2. 15A NCAC 02C .0108(p) — RESOLVED. Since the previous inspection, identification plates have been
maintained on the facility's environmental monitoring wells.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
1. On -site February 8, 2023, to observe the closed MSWLF cap and slopes, site access roads, facility environmental
monitoring wells, stormwater management/conveyance features, erosion control maintenance, and facility mowing
maintenance efforts. Facility records were reviewed before and after conducting the inspection.
2. Post inspection, additional facility research conducted which concluded on February 21, 2023.
3. Access to the site is located off Brushy Creek Road (S.R. 1101) and is controlled by a lockable gate.
4. Overall, access roads at the site were maintained. As discussed post -inspection, an eroded area was observed on a
small portion of the southern, central site access road (photo #1) which should be maintained. Continue to maintain site
access roads as necessary.

5. Edge -of -waste (EOW) markers were observed around the closed MSWLF and appeared to be maintained. Continue

to maintain EOW markers at the facility as necessary.
6. No apparent leachate breakouts or seeps were observed during this inspection.
7. Semi-annual groundwater and surface water monitoring reports for the facility dated February 2023 (for Fall 2022
sampling event) and September 2022 (for Spring 2022 sampling event) were available for review. Previous semi-annual
monitoring reports for the facility were also available for review. As required, continue to monitor all groundwater and
surface water sampling locations at the facility and submit reports to the Section in a timely manner.
8. The groundwater and surface water monitoring network at the facility is comprised of three groundwater monitoring
wells (MW-1, MW-2, and MW-3R), and two surface water monitoring locations (SW-1 and SW-2). During this
inspection, all groundwater and surface water monitoring locations at the facility were observed and access was
maintained for water quality monitoring locations at the site. Continue to maintain access for all water quality
monitoring locations at the site.
9. The facility's LFGCCS includes a Well Extraction Field with ten extraction points (designated #W-1 to #W-10), a
Flare Station, and Condensate Tank. The facility's LFG Monitoring Program includes thirteen monitoring well locations
(#P-1 to #P-13) around the perimeter of the landfill (located on and off -site), and six airport structures (located on the
adjacent Avery County Airport Authority property west of the facility). During this inspection, LFG monitoring
locations #P1 through #P-12 were observed, and access was maintained for LFG monitoring locations at the facility.
Continue to maintain access for all LFG monitoring locations at the facility. (Note: see Status of Past Noted Violations
#1 regarding monthly facility LFG monitoring and reporting.)
10. As discussed post -inspection, erosion has occurred at some drop inlet structures (photos #2 & #3) on the benches of
the upper slopes of the closed landfill. These drop inlet grates should be cleared of vegetative debris and any eroded
areas around the structures should be maintained as necessary to ensure positive flow to slope drains and outlet
structures at the facility. Continue to maintain all other erosion control measures at the facility as necessary including
slope drains, rip -rap channels, sediment detention basins, permanent diversions, and inlet/outlet stabilization structures.

11. During this inspection, ground cover appeared to be well established on the slopes and cap of the closed MSWLF.
Also, as discussed some areas of brush were observed on the lower slopes of the closed MSWLF during this inspection
and should be removed as needed (photos #4 & #5). After mowing events occur at the facility, continue to ensure the
landfill cap and side slopes are inspected for impounded water, subsidence/differential settling, cracking, signs of
erosion or damage, and that any necessary repairs are conducted as needed.
