(If additional space is needed, use back of this form) NAME OF FACILITY: Gallins Family Farm PERMIT # 3005 DATE AND TIME OF FIRE: 10/30/2022 @ 1:00 AM HOW WAS THE FIRE REPORTED AND BY WHOM: The fire was reported to the Farmington Rd. Fire Department by Lyndsy Gallins of Gallins Family Farm. LIST ACTIONS TAKEN: Fire break was created to prevent fire spreading and water was applied until the fire department arrived. WHAT WAS THE CAUSE OF THE FIRE: Due end of windrow was dry and with the dry weather there was spontanous combustion, which caught the shed on fire. DESCRIBE AREA, TYPE, AND AMOUNT OF WASTE INVOLVED: The area was the covered pole shed which houses the daily intake. The back of the shed is compost which is a little further long and drier. WHAT COULD HAVE BEEN DONE TO PREVENT THIS FIRE: The compost in the back of the shed could have been moved out or had continous moisture applied during this dry season. DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS: Keep water on piles during dry spells and avoid piles getting too high. Gallins will not be taking any new material in for the foreseeable future.
