STATUS OF PERMIT:
6403-LCID-2020: Permit to Construct and Permit to Operate was issued July 13, 2020, and the Permit to Operate will expire
on July 13, 2025.
PURPOSE OF SITE VISIT:
Compliance Inspection.
STATUS OF PAST VIOLATIONS
None.
OBSERVED VIOLATIONS
None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1. The all-weather roadway had been completed around the LCID.
2. Signage has been posted at the LCID and at the entrance, near the CDLF.
3. The working face was small. A portion of the LCID had been covered with soil.
4. The waste stream appeared to be relatively clean. A plastic bag was observed within the waste stream. Mr. Barnes
stated that he would remove the plastic bag from the waste stream. Ensure that unacceptable wastes are removed
and properly disposed of prior to covering the waste.
5. No woody vegetation was observed within the LCID footprint. It is recommended that the vegetation within the
LCID be mown on a regular basis to prohibit the growth of the vegetation strong enough to puncture vehicle tires.
6. A stockpile of cover soil was observed to the south of the LCID. The stockpile of soil is used as cover for the LCID
and CDLF Phase 5.
7. The vegetation on the side slopes of the sediment basin have begun to become overgrown. Mr. Barnes stated that
the gear box on the mower has been damaged. Mr. Barnes stated that the gear box should be in today, and the
facility would be mown after the repairs have been completed. Grassy vegetation was also observed within the
sediment basin. Continue to monitor and maintain the basin to ensure that the sediment basin is functioning
properly.
