PURPOSE OF SITE VISIT: Pre-Operations Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 8 ADDITIONAL COMMENTS: 01 – This facility inspection was conducted for a pre-operational inspection of the Wilson County Westside Construction and Demolition Debris Landfill Area 2, Phase 1. 02 – FYI: Permit to Construct for area 2 issued August 19, 2022, page 2 has the wrong Permit number (0403-CDLF-2004). Permit to Operate Data Table Permit Issuance Expiration 0403-CDLF-2004 August 19, 2022 September 17, 2064 03 – The site maps were reviewed and construction looks good. 04 – Edge of waste markers have been installed and are in the correct locations. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 8 05 – The stormwater diversion appears to be as drawn on the map and looks good. 06 – The Sedimentation Basin 1 appears to have been constructed as the drawing in the Erosion and Sedimentation Control Plan. 07 – The Basin 1 has the emergency spillway constructed with large gravel and has vegetation growing on the dike walls. Minor erosion on the Basin side. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 4 of 8 08 – The Basin 1 also has the discharge pipe installed. 09 – The roads are constructed of All Weather Construction (Asphalt/Gravel), no approved A-1 glass fines used at this time. 10 – Monitoring wells 4&5 were observed to be locked and tagged with required information. Hard to read. May want to replace tag. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 5 of 8 11 – Silt fencing is around Area 2, phase 1. 12 – The exterior of the total berm on the cell has been seeded and has vegetation up. May have to re-seed a few areas. 13 – Minor areas of erosion to be repaired on a couple of areas of the interior & exterior of the cell. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 6 of 8 14 – Currently, Protection of the Adjacent Archaeological Area consist of the silt fencing on the Landfill side and the facility is going to add metal (DOT) fencing on the (East) asphalt roadside with signage as well to prevent any disturbance of this area. 15 – A letter of Intent to Close will be sent to close the existing C&D cell and the existing LCID. Hopefully early next week. 16 – The site wants to use a temporary area to store LCID until the current area is closed and a new location is approved. Plenty of good area there to do this if permitting approves this. 19 – Digital pictures were taken during this inspection with some attached to this report.
