Expires July 11, 2023. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: NA The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS 1. David Powell with the Section inspecting Hoke County landfills. Met with April King, representing Hoke County as Hoke County Solid Waste Director. Ms. King walked with Section staff during inspection. Site has an LCID, 2-acre, approved 2020, YW staging and processing area and an animal burial unit. 2. Records reviewed at Hoke County consisted of: Transfer Station Assessment Report and response, Permit and operations plan were onsite; Previous inspections, 2021 Compost Time/Temps; Receipt for removal of mulch by 401 Sand; Daily tonnage logs; Omni Source receipt for WG and steel removal; 2021/2022 Oil removal receipts by Key Energy; 2021/2022 Safety- Kleene Systems oil removal records; 2021/2022 Carolina Tire removal receipts; 2022 Scrap Tire Certifications; 2022 Waste Screening Reports; Groundwater sampling report for 8/2022. Certified Operator Certifications: Gideon Locklear TRANS Expires 9/22/2024; Charles tedder TRANS Expires 9/22/2024; Brandon McKethan 3/9/2024; April King TRANS Expires 4/17/2025; Ms. King will be attending MOLO in near future. 3. YW has been stockpiled on western side of road access. Fire breaks are needed. Unapproved wastes very minimal during inspection. Traffic control measures were being implemented as well as signage explaining acceptable wastes and unacceptable wastes. Ms. King indicated it was time for mulcher company to come. Be sure and put fire breaks and to keep within the approved map area of 2 acres, approved in 2020. Edge of Waste (EOW) markers are in place in couple areas, however the limits of the 2-acre approved area should be marked with permanent markers and meet buffers. A surveyor or engineer can be utilized to ensure proper placement according to approved plans/maps/buffers. Recommend large white PVC pipe. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 5 4. Do not allow leachate to runoff from area designated for Yard Waste Debris Processing. Also, remember waste cannot be in water, so manage accordingly. Site should be graded to allow for positive drainage of water away from the waste storage and processing area. When this material is mulched and moved, take advantage of the time to buildup these areas and slope correctly. 5. If any YW is to go out to the public, then it must meet 15A NCAC 13B .1406 temperature rules. Be sure and log the temperatures and keep in logs for future inspections. A lot is taken by 401 Sand for erosion control and bank stabilization on mining slopes. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 4 of 5 6. LCID has had most waste covered but still needs additional cover. Solid waste must be covered with 1-foot of suitable soil cover at least once per month or when the active area reaches 1 acre in size, whichever occurs first, or more often when necessary to prevent the site from becoming a nuisance or to mitigate conditions associated with fire, windblown materials, vectors, or excessive water infiltration. Additional soil was present for cover and addressing any erosion. No unapproved wastes were visible. Good job. Working face, need additional cover, sloping. 7. Shrubs and trees should be removed on older cells and anywhere inside limits of the disposal area. EOW markers needed updating and to be installed around the entire the limits of LCID, all cells. Any waste disposal outside of the approved area or inside buffers should be removed inside the appropriate area and properly, covered, compacted, and sloped 3:1. Older cell with tree, no EOW marker. Animal carcass area has two designated areas on an approved map. Be sure that the carcasses make it to those designated areas only. Marking these areas with EOW markers is also needed. 9. An attendant should be on duty at all times while the landfill/YW area is open for public use to assure compliance with operational requirements and to prevent acceptance of unauthorized wastes. It is recommended that facility staff accompany customers to the LCID, YW and the animal carcass areas when dumping to better screen waste to ensure only acceptable waste is received. This would limit unapproved wastes and add more control of where waste is placed onsite. This would also ensure waste it disposed in proper place and for the animal carcass area ensure proper depth and cover is applied. 10. Corrective actions are necessary as result of this inspection. Corrective actions should be completed within 30 days from the date of this inspection report sent. A follow up inspection will be conducted at that time or shortly after. Failure to resolve these matters may result in further enforcement action.
