The Landfill facility is located at 2400 NC Hwy 42 East, Wilson, NC 27893 and is owned and operated by Wilson County under State Solid Waste Permit No. 98-09. A Permit to Construct for Area 2 Phase 1 was issued by the North Carolina Division of Waste Management (NCDWM) on August 19, 2022. 2.0 PROJECT DESCRIPTION 2.1 General Area 2 Phase 1 is an approximately 4.9-acre C&D landfill unit designed by Smith Gardner, Inc. (S+G). The landfill subgrade was constructed using on -site soils that meet NC DEQ's required soil types (SM, SC, ML, CL, MH, and/or CH). In addition to the landfill subgrade, Area 2 Phase 1 also includes associated perimeter berms and erosion and sediment control (E&SC) measures. 2.2 Reference Documents Area 2 Phase 1 was constructed in accordance with the following documents: Permit to Construct Application - Westside C&D Landfill - Area 2: Includes technical specifications, CQA manual, and construction drawings prepared by S+G and dated July 2020, revised June 2022. Permit to Construct - Westside C&D Landfill - Area 2: Issued by NCDEQ-DWM on August 19, 2022 (copy provided in Appendix A). Westside C&D Landfill - Area 2 Phase 1 - Erosion and Sediment Control Plan: Project ID WILSO-2022-012 submitted by Smith Gardner, Inc., April 29, 2022. Approved by the North Carolina Division of Energy, Mineral, and Land Resources (NCDEMLR) on June 27, 2022. Photos documenting the construction of Area 2 Phase 1 can be found in Appendix B. Prior to BJC beginning work, a project pre - construction meeting was held on June 3, 2022, a meeting summary is provided in Appendix C. 3.1 Site Preparation Construction began in June 2022 with the surveying/staking of the limits of construction and the installation of temporary erosion and sediment control measures (i.e., silt fence, stone outlets, etc.) surrounding the clearing limits. Following installation of these temporary measures, BJC initiated clearing and grubbing activities and installation of SB-1. 3.2 Erosion and Sediment Control Measures The construction of erosion and sediment control measures took place in conjunction with project activities and under the site permit issued by NCDEMLR. Permanent drainage channels and sediment basins were added as areas were brought to grade. Once areas reached final grade, the areas were vegetated in accordance with project requirements. 3.3 Excavation and Embankment Once portions of the site were cleared and grubbed, excavation and stockpiling activities were performed. Excavated materials including stripped topsoil, unsuitable soils, and structural fill were identified and separated. The soils within the Area 2 Phase 1 footprint identified for use as structural fill were excavated and placed and compacted where needed. Upon completing the Area 2 Phase 1 subgrade construction and in accordance with the permit, Mr. Seth Rickerts, P.G. visited and examined the site to identify any subgrade conditions that may adversely affect the design, construction, oroperation of the unit. As documented in Mr. Rickert's report (Appendix D) no areas of rock or any other unusual geologic features were identified. The subgrade consisted of sandy silts with varying amount of clays, meeting NCDEQ's requirements for a C&D landfill subgrade. 3.4 Well Abandonment On September 7th, 2022, Smith Gardner, Inc. personnel met drillers from Geologic Explorations to abandon PZ-1. This piezometer was over -drilled to remove the casing and the resulting We was grouted from bottom to top using a tremie-pipe to emplace the grout. A letter summarizing well abandonment activities which includes abandonment records is included in Appendix E. Area 2 Phase 1- Westside CBD Landfill Construction Quality Assurance Report September 2022 Page 3 DocuSign Envelope ID: 2AB7AE48-9C15-410A-9773-5308AA9F80BF 4.0 CQA PROGRAM 4.1 Scope of Services In satisfying the requirements of the Project CQA Manual for Area 2 Phase 1, the following activities were performed: • Observation and documentation of construction of prepared subgrade, groundwater underdrain, structural fill, and landfill subgrade; • Field and/or laboratory testing of structural fill; • Review of submittals from the Contractor for conformance with project specification and CQA requirements; • Review/preparation of record drawings; and • Preparation of the final CQA report. 5.0 EARTHWORK CQA The criteria for construction of structural fill per the project specifications included the following: Materials: SM, SC, ML, or CL (ASTM D 2487) with no topsoil or other deleterious material and no stones or rocks in excess of one half the lift thickness as compacted; Density: Minimum 95% Maximum Standard Proctor Dry Density (100% under roads and structures) (ASTM D 6981; Moisture Content: As necessary for compaction (+/- 2% of Optimum under roads and structures); and Lift Thickness: 8-inch max. (compacted). The number and results of material control and record tests performed on the structural fill are summarized in Table 1. Other tests performed on an on -going basis during construction included a visual classification of soils (ASTM D 24881 and monitoring of loose lift thickness. Note that the number of tests required was based on an approximate quantity of 9,400 CY of material placed (in -place measure). The on -site material used during Phase 1 were consistent with previous material testing and are representative of the materials used. The results of field and laboratory testing of structural fill can be found in Appendix F. 6.0 RECORD DRAWINGS The record drawings for Area 2 Phase 1 are included in Appendix G. The Project CQA Manual; ii. The conditions of the Area 2 Permit to Construct; iii. The requirements of 15A NCAC 13B.0541; and iv. Acceptable engineering practices. PART I GENERAL FACILITY This permit is issued by the North Carolina Department of Environmental Quality, Division of Waste Management, Solid Waste Section (Section). In accordance with North Carolina Solid Waste Management Rule 15A NCAC 13B .0201(d), a solid waste management facility permit shall have two parts: a permit approval to construct and a permit approval to operate. The permit approval to construct must be implemented in accordance with Attachment 2 of this permit. The permit approval to operate must be implemented in accordance with Attachment 3 of this permit. 2. The persons to whom this permit is issued ("permittee") are the owners and operators of the solid waste management facility. 3. The Permit to Operate dated January 21, 2016, was recorded at the Wilson County Register of Deeds on February 17, 2016, in Deed Book 2645, pages 381-399. 4. When this property is sold, leased, conveyed, or transferred in any manner, the deed or other instrument of transfer must contain in the deed description section, in no smaller type than that used in the body of the deed or instrument, a statement that the property has been used as a sanitary landfill and a reference by book and page to the recordation of the permit. 5. By beginning construction or receiving waste at this facility the permittee shall be considered to have accepted the terms and conditions of this permit. 6. Construction and operation of this solid waste management facility shall be in accordance with the Solid Waste Management Rules, 15A NCAC 1313; Article 9 of Chapter 130A of the North Carolina General Statutes (N.C.G.S. 130A-290, et seq.); the conditions contained in this permit; and the approved plan. Should the approved plan and the rules conflict, the Solid Waste Management Rules shall take precedence unless specifically addressed by permit condition. Failure to comply may result in compliance action or permit revocation. 7. This permit is issued based on the documents submitted in support of the application for permitting the facility identified in Attachment 1, "List of Documents for Approved Plan," which constitute the approved plan for the facility. Where discrepancies exist, the most recent submittals and the conditions of permit shall govern. 8. This permit may be transferred only with the approval of the Section, through the issuance of a new or substantially amended permit in accordance with applicable statutes and rules. 9. The permittee is responsible for obtaining all permits and approvals necessary for the development of this project including approval from appropriate agencies for a General or Individual National Pollutant Discharge Elimination System (NPDES) Stormwater Discharge Permit. Issuance of this permit does not remove the permittee's responsibilities for compliance with any other local, state, or federal rule, regulation, or statute. The initial, substantial, construction authorized by this Permit Approval to Construct must commence within 18 months from the issuance date of the permit issued August 19, 2022. If substantial construction does not begin by this date, then the permit to construct shall expire. Substantial construction includes, but is not limited to, issuance of construction contracts, mobilization of equipment onsite, and construction activities including installation of sedimentation and erosion control structures. The permittee must conduct a preconstruction meeting at the facility prior to initiating construction of any unit/cell and must notify the Section at least 10 days prior to the meeting. 3. Construction of all solid waste management units within this facility must be in accordance with the approved plans and only for those phases of development approved for construction as described in the list of approved documents in Attachment 1. 4. Modifications or revisions of the approved documents or changes during construction of any landfill unit/cell require approval by the Section and may constitute a permit modification. 5. Construction of groundwater monitoring wells and landfill gas monitoring wells, and abandonment of groundwater and landfill gas wells, shall be in accordance with the conditions of Attachment 3, Environmental Monitoring Conditions. 6. Modifications to the approved sedimentation and erosion control activities require approval by the NC Division of Energy, Mineral and Land Resources. The Section must be notified of any modifications. 7. Burning of land -clearing debris generated on site as a result of construction activities may be allowed only if prior to initiating the controlled burning, the permittee obtains approvals and/or permits from the Section, the NC Division of Air Quality in accordance with 15A NCAC 02D .1900, the NC Forest Service, and the local government. The permittee must conduct the approved open burning in compliance with all air pollution and open burn laws, regulations, and ordinances. 8. The following requirements must be met prior to operation of any newly constructed area: a. Construction Quality Assurance (CQA) documentation and a certification by the project engineer that the unit was built in accordance with approved plans and the conditions of the permit must be submitted to the Section for review and approval. b. The edge of waste footprint must be identified with permanent physical markers. The Permittee must contact the regional environmental specialist and permitting engineer to determine whether the Section chooses to hold a pre -operative meeting with key landfill personnel and representatives of the Section. d. Documentation of financial assurance mechanisms must be submitted to the Section, as applicable. The financial assurance amount must include closure and post -closure costs including the new area to receive the permit to operate. Life -of -site ends when the facility reaches its final permitted elevations, or no later than September 17, 2064, which is 60 years from the initial operation of the landfill. Continued operation after this date will require a permit renewal in accordance with rules and requirements at that time. 2. The facility is approved to operate Area 1. Construction and demolition debris, defined in 15A NCAC 13B .0532, means solid waste generated solely from the construction, remodeling, repair, or demolition operations on pavement and buildings or structures. C&D waste does not include municipal and industrial wastes that may be generated by the on -going operations at buildings or structures. b. Inert debris, defined in 15A NCAC 13B .101, means inert debris that consists solely of asphalt, cured concrete, brick, concrete block, gravel, and rock. Inert debris waste shall not contain chemical adhesives or sealants, or lead -based paint. C. Land -clearing waste, defined in 15A NCAC 13B .101, means land -clearing debris that consists solely of stumps, trees, limbs, brush, grass, and other naturally occurring vegetative material. 4. Regulated asbestos -containing material as defined in 40 CFR 61 must be managed in accordance with 40 CFR 61. Disposal of asbestos waste must be in accordance with Rule 15 NCAC 13B .0542 and the approved operations plan, and must be only disposed within the C&D landfill. 5. Wastewater treatment sludge is not approved for disposal. Wastewater treatment sludge may be accepted, with the approval of the Section, for utilization as a soil conditioner and incorporated into or applied onto the vegetative growth layer. The wastewater treatment sludge must not be applied at either greater than agronomic rates or to a depth greater than six inches according to Rule 15A NCAC 13B .0542. 6. Those wastes listed in Rule 15A NCAC 13B .0542 (e) must not be accepted for disposal, including, but not limited to, hazardous waste, municipal solid waste (MSW), liquid waste, commercial or industrial wastes, and yard trash. 7. Wooden pallets and yard waste are not approved for disposal. Wood pallets and yard waste may be accepted and managed in the on -site Wood Processing and Yard Trash Composting Area according to the approved plan. 8. This facility is permitted to receive solid waste generated within Wilson County consistent with the resolution passed by the Wilson County Board of Commissioners on June 1, 2020. 9. The facility is approved to accept wastes at a rate of maximum 50,000 tons per year consistent with the resolution passed by the Wilson County Board of Commissioners on June 1, 2020. Maximum variance shall be in accordance with NCGS 130A-294(bl)(1). 10. The permittee must not knowingly dispose of C&D waste that is generated within the boundaries of a unit of local government that by ordinance: a. Prohibits generators or collectors of C&D waste from disposing of that type or form of C&D waste. b. Requires generators or collectors of C&D waste to recycle that type or form of C&D waste. 11. The permittee must actively employ a training and screening program for detecting and preventing the disposal of excluded or unauthorized wastes. Random inspections of incoming loads or other comparable procedures. b. Records of any inspections. C. Training of personnel to recognize hazardous, liquid, and other excluded waste types. d. Development of a contingency plan to properly manage any identified hazardous, liquid, MSW, or other excluded or unauthorized wastes. The plan must address identification, removal, storage, and final disposition of these wastes. 12. In accordance with NCGS 130A-295.6, this landfill may use alternative daily cover (ADC) that has been previously approved at another sanitary landfill in North Carolina. The Section maintains a list of approved ADC and its appropriate use, which may be referred to, but is not required to be, in determining ADC types and uses. a. The use of ADC that does not meet the condition above requires approval of the Section prior to implementation. Requests for ADC approval must include a plan detailing the comprehensive use and a demonstration of the effectiveness of the alternative cover, developed according to Section guidelines. Plans that are approved by the Section will be added to the list of approved documents listed in Attachment 1. b. The permittee must maintain records for all solid waste materials accepted and used as ADC. The records must include: the date of receipt, weight of material, general description of the material, identity of the generator and transporter, and county of origin. Records must be made available to the Section upon request. 13. Routine inspections of cover for leachate seeps and erosion, and repair, shall be conducted in accordance with the approved plan. 14. Financial assurance must be continuously maintained for the duration of the facility in accordance with applicable state rules and statutes. Closure and Post -Closure cost estimates and financial instruments must be updated annually. Environmental Monitoring Conditions 15. Groundwater, surface water, and landfill gas monitoring locations must be established and monitored according to state rules and the approved plans. Any proposed modification to an approved plan must be submitted to the Section and approved prior to implementation. 16. The permittee must obtain approval from the Section for the design, installation, development or abandonment of any groundwater monitoring well or landfill gas monitoring well. Any modification to the approved Water Quality Monitoring Plan or Explosive Gas Monitoring Plan must be reviewed and approved by the Section. 17. Groundwater monitoring wells and surface water monitoring points must be sampled at least semi-annually in accordance with state rules, the approved water quality monitoring plan, and the current policies and guidelines of the Section in effect at the time of sampling. In accordance with Rule 15A NCAC 13B .0544 and the approved landfill gas control plan, landfill gas monitoring must be conducted quarterly, unless otherwise specified by the Section. Reports of the analytical data for each water quality monitoring event must be submitted to the Section within 120 days of the sample collection date. The permittee must provide a potentiometric surface map for the current sampling event that shows groundwater and surface water sampling locations, and all new, existing, and abandoned groundwater well locations. Analytical data must be submitted in a manner prescribed by the Section. 19. A readily accessible unobstructed path must be maintained so that groundwater and landfill gas monitoring wells and surface water sampling locations are accessible using four-wheel drive vehicles. 20. Documentation of well completion, well development details, repair, abandonment, and all other pertinent activities associated with each groundwater and landfill gas monitoring well must be maintained in the facility operating record. The permittee must maintain a record of all groundwater, surface water, and landfill gas monitoring events and analytical data in the operating record. 21. All landfill gas monitoring events must be conducted quarterly by properly trained personnel and must include monitoring for all explosive gases, including hydrogen sulfide according to the approved monitoring plan. Landfill gas monitoring must include interior monitoring of onsite buildings. 22. Landfill gas monitoring results must be recorded on forms provided by the Section. Erosion and Sedimentation Control Requirements 23. All earth disturbing and sedimentation and erosion control activities must be conducted in accordance with the Sedimentation Control Act N.C.G.S. 113A-50, et seq., and rules promulgated under 15A NCAC 4. All required sedimentation and erosion control measures must be installed and operable to mitigate excessive on -site erosion and to prevent silt from leaving the area of the landfill unit during the service life of the facility. 24. Modifications to the approved sedimentation and erosion control activities require approval by the North Carolina Land Quality Section. The Section must be notified of any sedimentation and erosion control plan modifications. 25. Facility construction, operations or practices must not cause or result in a discharge of pollution, dredged material, and/or fill material into waters of the state in violation of the requirements under Sections 402 and 404 of the Clean Water Act, as amended. Reporting and Recordkeeping Requirements 26. Copies of this permit, the approved plans, and all records required to be maintained in the operating record by the permittee must be maintained at the facility, unless otherwise approved by the Section, and made available to the Section upon request during normal business hours. 27. Scales must be used to weigh the amount of waste received. The daily records are to be summarized into a monthly report for use in the required annual reports. The permittee must maintain records of: a. The amount of all accepted solid waste materials as (i) C&D wastes, (ii) alternative cover material used as alternate periodic cover, and (iii) recyclable material. Daily records of waste received, and origins of the loads. 28. On or before August 1 annually, the permittee must submit an annual facility report to the Section, on forms prescribed by the Section. a. The reporting period shall be for the previous year beginning July 1 and ending June 30. b. The annual report must list the amount of waste received and landfilled in tons and be compiled: i. On a monthly basis. ii. By county, city or transfer station of origin. iii. By specific waste type. iv. By disposal location within the facility. V. By diversion to alternative management facilities. C. A measurement of volume utilized in the C&D cells must be performed during the second quarter of the calendar year. The date and volumes, in cubic yards, must be included in the report. d. The amount of C&D waste, in tons from scale records, disposed in landfill cells since September 17, 2004, through the date of the annual volume survey must be included in the report. e. The completed report must be forwarded to the Section's Regional Environmental Senior Specialist for the facility by the date due on the prescribed annual facility report form. f. A copy of the completed report must be forwarded to each county manager for each county from which waste was received at the facility. Documentation that a copy of the report has been forwarded to the county managers must be sent to the Regional Environmental Specialist by the date due on the prescribed annual facility report form. Closure and Post -Closure 29. The permittee must conduct closure and post -closure activities in accordance with the approved plan and Rule 15A NCAC 13B .0543. Prior to beginning closure of each C&DLF unit, the owner must notify the Division in writing that a notice of the intent to close the unit has been placed in the operating record. 30. The owner shall submit an application for a closure and post -closure care permit to the Division when the facility reaches its final permitted elevations and prior to initiating closure activities for the final permitted C&DLF unit at the facility. PART II: MISCELLANEOUS SOLID WASTE MANAGEMENT ACTIVITIES General Conditions 31. Wastes and recyclable products, as described below, shall be managed in the designated areas as shown on the approved application drawings. The permittee must obtain Section approval before re -locating any of these operations or revising the operations. Wastes and recyclables must be stored in reasonably sized piles with adequate fire breaks and lanes. 33. Surface water shall be diverted from all operational and storage areas to prevent standing water in operational areas and under or around storage piles. Water that comes in contact with solid waste is deemed to be leachate and shall be contained on -site or properly treated prior to discharge. 34. Sufficient dust, odor, and litter control measures must be employed to minimize airborne emissions and to prevent nuisances. 35. Effective vector control measures shall be applied as necessary to control flies, rodents, insects, and vermin. 36. The waste tire collection area shall be operated in accordance with the requirements of Rule 15A NCAC 13B .1107 and the approved operations plan. 37. White goods containing refrigerants must be stored upright and handled in a manner to prevent leakage. Refrigerants must be properly removed by trained personnel prior to removal off -site for recycling. Yard Waste and Wood Waste Processing Area 38. The facility operation includes a 4.5-acre Wood Processing Area, and small Type 1 composting operation. 39. The operation is approved to accept land clearing debris, yard waste, and untreated and unpainted wood waste/pallets. Maximum storage at any given time is 6,000 cubic yards. Larger storage amounts will require a permit application for a Large Type 1 facility. 40. Pile sizes for unprocessed and processed wood material (mulch) must be limited to 50 feet wide, 30 feet high, with unlimited length within the approved wood waste/composting boundaries, unless otherwise approved by the Section. Active compost pile sizes are limited to 10 feet high and 15 feet wide, unlimited length within the operation boundaries, unless otherwise approved by the Section. 41. Unless used for boiler fuel, yard trash must be composted to meet pathogen reduction, in accordance with Rule 15A NCAC 13B .1406. Written records must be maintained to document the compost process reaching 131 degrees for three days for each batch or pile. Inert Debris Disposal Area 42. Inert debris disposal at the site, must meet either the requirements listed in 15A NCAC 13B .0562 for beneficial fill, or the requirements listed in 15A NCAC 13B .0563 et seq., for a land clearing and inert debris landfill. Glass Fines for Road Material 43. The landfill is permitted to accept and use residual glass/porcelain A2 Fines as road material inside the CDLF waste footprint according to the approved plan (DIN 25352). The only approved source of the material is Reflective Recycling, Inc., address 2606 Wilco Boulevard, Wilson, NC 27893, Phone 281-520-0613. The permittee must conduct random waste screening according to the approved operations plan (DIN 25352) to ensure that the material is not co -mingled with non- conforming waste material by more than 10% volume for each load. 45. The maximum quantity of the residual glass/porcelain A2 Fines that can be used as road material is 500 tons per year. 46. The material must be stored within the landfill footprint and used at approximately one month intervals. Skimmer outlet/rip-rap apron is being used as sediment and erosion control measure for this project. If you are going to use existing basin that was previously approved, for a new project, you must include all parts of the basin, including the skimmer outlet/rip-rap apron within the LOD of the new project. In our experience, it has been observed that silt fence and silt fence outlet below skimmer outlet is very effective in preventing offsite sediment. You can choose not to add the silt fence and outlet below the skimmer outlet on the plans but if during inspection offsite sediment is noted in this area by our inspector, the contractor will be asked to install additional measures. What is the purpose of adding the dewatering silt bag if the basin has been previously established and the skimmer can be installed in the dry without dewatering? Once the skimmer is installed, there will be no need to dewater through a silt bag? Dewatering silt bag is also needed for emergency maintenance purposes. Also, have the baffles already been installed in basin? Dewatering silt bag could be used to drain the basin to install the baffles if they haven't been installed already. On September 7, 2022, S+G performed an inspection of the subgrade. The inspection indicated that the cell was substantially complete. During the inspection, the subgrade soils within two feet of the subgrade surface were visually classified as fine sandy silts with varying amounts of clay. No soils that were predominantly sand were found during this inspection, nor was bedrock or any other unusual geologic feature. Based on the observations made observations, S+G certifies that the subgrade soils are materially consistent with the soil types found during the Area 2 permitting investigation. A map of the piezometer location is shown on Figure 1. Abandonment Overview The piezometer PZ-1 was located within the footprint of the Wilson County landfill Area 2 Expansion. In accordance with 15A NCAC 02C .0113, S+G personnel mobilized to the facility on 09/07/2022 to oversee the abandonment performed by Geologic Explorations Inc. (GEX). Abandonment procedures included removing the piezometer's outer steel casing and PVC stickup, overreaming the existing borehole with 4.25" hollow stem augers to depth (20' bgs), and tremie grouting the borehole from the bottom to the ground surface. After abandonment, the piezometer's outer steel casing and PVC stickup were disposed of properly. The piezometer abandonment record is included in Attachment A.
