Expires August 09, 2021. Pursuant to 15A NCAC 13B .0201(g), no later than April 27, 2021, the owner or operator must submit a request to the Section for permit review and must submit updated facility plans meeting the requirements of Rule 0.565 and .0566. Renewal Packet has been sent to DWM. Additional information has been requested from DWM dated 10/26/2021 and has been sent to DWM. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 4 ADDITIONAL COMMENTS: 01 – Facility has signage. Need to update with all required information below. Per Last Inspection: Has all but the below red highlighted info. 15A NCAC 13B .0566 OPERATIONAL REQUIREMENTS FOR LCIDLFS. (Rule attached to the email.) (14) A sign shall be posted at the site entrance showing the site contact's name and phone number, the permit number, emergency contact information, and the waste types accepted for disposal at the site. Needed: Change contact name. (Brent to Alicia) and phone number. 02 – Locking gates and fencing are used for security. 03 – Roads are constructed of all-weather materials and maintained well. 04 – Edge of waste markers present. 05 – Cover logs were not reviewed during this inspection. Most of the LCID has been covered recently. 06 – Adequate amount of earthen cover material on-site and ready for use. 07 – Facility is meeting cover requirements that ALL waste must be covered within 30 days. 08 – The issue of non-conforming waste was not an issue during this inspection. 09 – Facility has done a lot of grade work on the side slopes. 3:1 is now most of the landfill. Looks good. 10 – Incoming load receipts were not reviewed during this inspection. Please update records and have available for the next comprehensive facility compliance inspection. 11 – Most of the sapling trees have been removed. 12 – Please continue to inquire (emails) the Permitting Staff at DWM about your expired permit status. 13 – This permitted facility is not open to the public. May want to add to the sign also, but not required. 14 – Digital pictures were taken during this inspection with several attached to this report. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
