STATUS OF PERMIT:
Permit to Operate issued November 14, 2021; Permit expires April 11, 2021. Permit renewal application submitted
and currently under review.
PURPOSE OF SITE VISIT: Comprehensive Inspection

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. This facility is a Wood Waste Treatment & Processing Facility, Permit No. 6511-TP. It vegetative land clearing
debris, root mat, stumps, limbs, tree trunks, soil, and shrubbery.
2. The facility grinds land clearing debris for use as boiler fuel and mulch.
3. The permit, operations plan, and site plan were reviewed with the facility operator.
4. The operating hours are Monday through Friday 8:00 AM - 5:00 PM.
5. Signs posted indicating hours of operation and waste exclusions.
6. No offsite sedimentation was observed.
7. An interior roadway has been made of all-weather construction and is being maintained in good condition.
8. There is at least a 25 foot access path around the site for fire control vehicles.
9. All setbacks are in place.
10. No standing water was observed during the inspection
