Permit Modification #1, Permit to Operate issued February 1, 2000. Permit Amendment, Permit to Construct & Operate issued March 28, 2013. A Life of Site Permit was issued May 8, 2019. The Permit will expire on May 8, 2069. PURPOSE OF SITE VISIT: Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. Page 1 of 3 NORTH CARnLINAD_E Q�� o.n.mm.m m c.w.mmen.i a.i� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. The facility is fenced and is secured by a locking gate when it is not open for operation. 2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as materials accepted at the facility. 3. The facility accepts waste Monday through Saturday from 7:30 am until 4:30 pm, with the exception of Wednesday 7:30 am until 12:00 pm. 4. The transfer station walls were covered with dirt and grime and are in need of cleaning. As per 15A NCAC 13B .0405 (11), the transfer station building shall be cleaned with a pressure washer no less than twice per year. Ensure that the transfer station is cleaned no less than twice per year. 5. Damage to the sheet metal above the transfer trailer loading bay. The sheet metal should be monitored and repaired as needed as identified in the Transfer Station Assessment Report. 6. Leachate was found to be pooling in the exterior leachate drains. No waste was observed in the exterior leachate drains. Mr. Holland stated that the leachate drains had recently been cleaned. Mr. Holland also stated that there was some damage to the leachate pipe to the leachate tank. Ms. Thompson stated to Mr. Holland that the leachate drain needed to be repaired so that the leachate can flow freely. Ensure that the drains are repaired and cleaned as needed so that the leachate can flow freely into the collection tank. 7. At the time of this inspection, windblown waste was observed outside of the transfer station building, within the trailer loading area, and within the trailer staging area. All of the windblown waste must be collected and returned to the transfer station urior to close of business daily. 8. Due to the busyness of the transfer station during this inspection, the condition of the tipping floor could not be observed. However, Mr. Holland stated that the condition of the tipping floor had not improved since the last inspection. The replacement of the tipping floor has been sent out for bid. During this inspection, several large areas of cracks and holes were observed in the tipping floor. Some of the co Monitor the tipping floor daily to ensure that the holes do not grow in size and that leachate is not coming in contact with the soils below the tipping floor. Modification of where waste is placed on the tipping floor may be required to avoid the damaged areas. The sampling of soils beneath the tipping floor may be required to determine if leachate has come in contact with the soils. 9. 10. The white goods were stored upright on the concrete.
