HOW WAS THE FIRE REPORTED AND BY WHOM:
3ordon Dively called Susan Heim and Jaclynne Drummond to report the incident.
LIST ACTIONS TAKEN:
The fire was extinguished with water that was present on site and buried with soil. The fire department was contacted and responded to the site as well. A call was
31aced to our landfill gas consultant to confirm the best course of action going forward.
WHAT WAS THE CAUSE OF THE FIRE:
kn active extraction well was in the process of being raised. Once the well head was removed, the top plate of the caisson well was being removed to fill the annular
>pace with gravel prior to pulling the outer casing up when, it is believed, that the nut spun and created a spark.
DESCRIBE AREA, TYPE, AND AMOUNT OF WASTE INVOLVED:
Vo waste was involved.
WHAT COULD HAVE BEEN DONE TO PREVENT THIS FIRE:
Use of breass sockets to negate the possiblities of a metal on metal spark.
DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS:
The contractor is purchasing brass sockets for future well raising events.
