One Category 1 MSW Cell and two Category II MSW Cells per the "Landfill Closure Plan" submitted by Jensen Engineering, Project No. 93131 (P.E. signed October 6, 1994) and the revised "Landfill Closure Plan Drawing No. 93131" dated October 19, 1994, submitted by Jensen Engineering. PURPOSE OF SITE VISIT: Comprehensive Site Inspection. STATUS OF PAST NOTED VIOLATIONS: 1. 15A NCAC 13B .0503(2)(a) CORRECTIVE MEASURES ONGOING. Since the previous facility inspection, Madison County submitted an Updated Landfill Gas (LFG) Monitoring Plan (FID 1236572) to address LFG exceedances at the facility. As discussed post -inspection with Solid Waste Director Mr. Sam Lunsford, please continue to coordinate with Section Hydrogeologist, Jaclynne Drummond, at email jaclynne.drummond ckncdenr..og_v, for ongoing facility LFG corrective action measures and submittals. OBSERVED VIOLATIONS: 1. 15A NCAC 02C .0108(p). At the time of this inspection, the facility has not demonstrated that the closed site has met the standards of construction in 15A NCAC 02C .0108(p) which states: "Each non -water supply well shall have permanently affixed an identification plate. The identification plate shall be constructed of a durable, waterproof, or rustproof material and shall contain the following information: Page 1 of 5 FACILITY COMPLIANCE INSPECTION REPORT �DHF 4� Division of Waste Management Solid Waste Section (1) well contractor's name and certification number; (2) the date the well was completed; (3) the total depth of the well; (4) a warning that the well is not for water supply and that the groundwater may contain hazardous materials; (5) the depth to the top and bottom of each screen; and (6) the well identification number or name assigned by the well owner." During this inspection, it was observed that identification plates were not maintained as required on environmental monitoring wells MW-01 OLD, MW-02 OLD, GP-6R, and sampling location BG SPRING OLD at the facility (see example photos #1 & #2). 1. To achieve compliance, Madison County must affix a permanent identification plate with the required information on environmental monitoring wells as needed at the Madison County Landfill Permit No. 5802-MSWLF-1981 as specified in 15A NCAC 02C .0108(p). The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. On -site March 15, 2022, to observe the closed municipal solid waste landfill (MSWLF) cap and slopes, site access roads, facility environmental monitoring wells, passive LFG vents, erosion control maintenance, and facility mowing maintenance efforts. Facility records were reviewed prior to conducting the inspection. 2. Post inspection, additional facility research conducted which concluded on March 25, 2022. Access to the site is located off Long Branch Road (SR 1582) and is controlled by a lockable gate. 4. Access roads at the site appeared to be well maintained. 5. Edge -of -Waste (EOW) markers were maintained at the facility. As discussed during post -inspection, continue to maintain all EOW markers at the site as necessary. 6. No apparent leachate breakouts were observed during this inspection. As discussed post -inspection, a very small potential seep (approximately l' x 1') was observed on the bench of the slope that is southwest of the stormwater inlet grate that was maintained since the previous inspection (photo #3). As discussed, inspect this area and maintain as necessary. (Note: the seep appeared to be within the EOW and the facility received a moderate amount of rain prior to the inspection.) Additionally, continue to monitor the closed landfill cap and slopes for any damp or wet areas and maintain as necessary. 3. Potential seep observed on bench of slope in central, southern area of facility (location discussed with SW Director). Note: ID badge included for scale. 7. No exposed waste was observed during this inspection. 8. Semi-annual groundwater, surface water, and domestic well monitoring reports for the facility dated February 21, 2021 (for Fall 2021 sampling event) and May 19, 2021 (for Spring 2021 sampling event) were available for review. As required, continue to monitor all groundwater, surface water, and domestic well sampling locations and submit reports to Section Hydrogeologist Jaclynne Drummond. 9. The groundwater, surface water, and domestic well monitoring network for the facility is currently comprised of four groundwater monitoring wells designated as MW-01 OLD, MW-02 OLD, MW-03 OLD, MW-04 OLD, one background Page 3 of 5 FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section groundwater quality sampling location designated as BG SPRING OLD, two surface water locations designated as SW- 01 OLD and SW-02 OLD, and a domestic water supply well designated as DW-0I OLD. Access appeared to be well maintained for all groundwater and surface water monitoring locations at the site. Continue to maintain access for all water quality monitoring locations at the site. (Note: Please see Observed Violation #1 of this inspection report for additional groundwater monitoring well information for the facility.) 10. 2021 quarterly LFG monitoring reports for the facility were available for review. Quarterly LFG monitoring reports for the facility for 2019 and 2020 were also available for review. As required, continue to monitor all LFG sampling locations quarterly and submit reports to the Section as necessary. (Note: Please see Status of Past Noted Violation #1 of this inspection report for additional LFG information for the facility.) 11. The LFG monitoring network at the facility is currently comprised of eleven wells designated as GP-01, GP-02, GP- 03, GP-04, GP-05, GP-06R, GP-07, GP-08, GP-09, GP-10, and GP-11. Indoor sampling locations for LFG are in the adjacent County Animal Shelter and County Transit Building situated along the eastern side of the facility and in the Madison County Sheriff's Office on the northern side of the site. Access appeared to be well maintained for all LFG monitoring locations at the site. Continue to maintain access for all LFG monitoring locations at the facility. 12. Continue to maintain PVC piping associated with the passive LFG vent system at the facility. 13. Since the previous inspection, the erosion gulley observed between the northern side of the Category I cell and the southeastern side of the northwestern Category II cell has been maintained. (Note: the erosion gulley appeared to be between these two cells and not over any waste area.) As observed on -site and as discussed with Mr. Lunsford post - inspection, the facility installed a stormwater inlet and grate with associated plastic piping and outlet protection (photos #4 & #5) to help control surface runoff which was likely contributing to the erosion in this area. The facility appeared to have also partially maintained the corrugated metal pipe which also helps control drainage in this area. As discussed, continue to observe this area and maintain as needed to ensure positive drainage at the site. Continue to maintain all other erosion control measures at the facility as necessary including rip -rap channels, sediment detention basins, permanent diversions, and inlet/outlet stabilization structures. 4. As discussed post -inspection, remove any trees, shrubs, and/or thick brush from all landfill side slopes as necessary. Additionally, as discussed, remove any trees and undergrowth in the former sediment basin(s) at the site. After mowing events occur at the facility, continue to ensure the landfill cap and side slopes are inspected for subsidence, cracking, signs of erosion or damage, and that any necessary repairs are made in a timely manner. 15.
