Expires 12/23/2057. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: Resolved, except for small pieces of trash. OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS: 01 - This facility was issued a Life of Site permit on April7, 2021 and expires on December 23, 2057. 02 - The Permit to Operate and Operations Plan were not reviewed during this inspection. 03 - This follow-up inspection (3/22/2022) was conducted due to non-compliance issues that were self-reported by Michael Lee - GFL Operation Manager on Tuesday, 2/15/2022, Inspection conducted on 2/21/2022 with observed violations found. 04 - Waste was being left on the tipping floor for several days. The Tipping Floor looked ok today. 05 - During the last inspection, several Observed Violations were noted below. (highlighted in red). 06 - Per last inspection: 15A NCAC 13B .0405 OPERATIONAL REQUIREMENTS: * (10) Windblown waste: Site staff shall conduct daily inspections for windblown waste on the site property. Windblown litter from site operations discovered during the daily inspections or observed on adjacent properties shall be picked up and containerized for disposal by the end of each operating day, unless the landowner of the adjacent property denies access to site staff. The windblown trash was minimal during operating hours. Still need to get the small pieces of waste raked or vacuumed up at the pond area and by the staff office and the visitor parking area at the entrance. Some of the small pieces are from the recent mowing, You could find out the contract mowers schedule and pick up any windblown trash from the morning to reduce the amount of smalls on the yard. . * (11) Site Cleaning and Maintenance: Unless otherwise stated in the site permit, all waste shall be removed from the tipping floor, the truck loading bays, and from behind push walls by the end of each day of operation and disposed of in accordance with this Subchapter. Overall, the tipping floor looked fairly clean during this inspection. Except for the leachate drain. 07 - The leachate drain on the west end (upper end of drain system) of the tipping floor was clogged up. Cover was removed by Stella Staff. Area of tipping floor behind this drain was very wet. Tire tracking a potential issue leaving the facility. 08 - This drain needs to be monitored and cleaned (multiple times a day) as needed to prevent this from happening to also prevent non-compliance of the facility. 09 - The middle areas of the leachate drain were ok. 10 - The east end of the leachate drain (recycle side) had solids clogging the grates, but the drain was flowing on this end. 11 - Some waste noted hanging out of metal on back side of building. None on the ground at this time. Repair/replace metal. 12 - There was only 2 loaded trailers at the stored trailer area. No leaks. 1 trailer had an issue with the rear door closing. Ricky Knight was moving this trailer back onto the tipping floor to repair. 13 - It appears that waste is now being removed from the facility at a normal pace to ensure no back ups occur. 14 - This facility is contracted to Stella Environmental Service for daily operations and hauling. 15 - Ricky Knight, Stella Environmental accompanied John College, DMW and Tyrone Sumler, GFL on this follow-up inspection. All items above were discussed. 16 - Digital pictures were taken during this inspection with some attached to the report.
