LOS Issued on January 22, 2020 and expires on June 14, 2054. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 4 ADDITIONAL COMMENTS: 01 - This facility has required signage and has locking gates at the entrance. Facility has fencing at tipping floor to prevent wind blown trash. 02 - The service area is Pasquotank and Camden Counties. 03 - The access roads are of all-weather construction. 04 - Waste is being transported to WM-Atlantic Waste Disposal Landfill in Waverly Va. or transported to East Carolina Regional Landfill (permit# 08-03) in Aulander, Bertie Co. 05 - Leachate system looks good. Leachate drains were clean during this inspection. 06 - Facility Certified Transfer Station Operations Specialist and 1 MOLO: Janice V. Brown – expires October 19, 2022 Rick O. Holley – expires August 28, 2024 Brad Gardner, MOLO, expires September 15, 2023 07 - No Windblown trash was noted at this facility during working hours and this inspection. 08 - Screening records were available and reviewed and found to be up to date. New 1/2021 rule requires that; 15A NCAC 13B .0405 OPERATIONAL REQUIREMENTS; (8) Waste Screening. Sites shall comply with the following waste screening requirements: (A) Site personnel shall screen incoming loads weekly at a rate of no less than five percent of the average daily waste tonnage reported in the site's annual report for the previous year. 09 - No leaks were found from stored trailers. Some needed repairs were pointed out to Brad Gardner during the inspection. 10 - The tipping floor was clean during this inspection. 11 - The trailer loading bay was free of waste and clean during operating hours. 12 - Scrap metal is pulled from the waste stream at this facility and is placed into containers for recycling. 13 - Operating hours for this facility are: Monday – Friday, 8am to 4:30pm and Saturday, 8am to 12pm. 14 - Scale tickets were reviewed during this inspection and has required information. 15 - The LOS Permit to Operate was issued 1/22/2020 and expires 6/14/2054. 16 - Permit and operations plan were available and reviewed. 17 - Leachate tanks have storage and removal records were available and reviewed. Last dated receipt was January 26, 2022. There may have been another removal and the receipt hasn’t been sent to the facility from Accounts Payable. 18 - Digital pictures taken during this inspection with several attached to this report. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
