(If additional space is needed, use back of this form) NAME OF FACILITY: Iredell County Solid Waste PERMIT # 4903 DATE AND TIME OF FIRE: 3/12/2022 @ 6:30 am HOW WAS THE FIRE REPORTED AND BY WHOM: Landfill employees reported fire to their supervisor Roger Eades. Roger Eades notified Director Teddy Boller. LIST ACTIONS TAKEN: Landfill employees used dozers to segregate the burning material from the waste mass %voter was applied with a hydroseeder. Finally, dirt was applied to smother the fire. WHAT WAS THE CAUSE OF THE FIRE: rherc was no determination of what started the fire. it is our assumption the fire was started by a lithium battery. DESCRIBE AREA. TYPE, AND AMOUNT OF WASTE INVOLVED: The fire was in an area on the back side of phase 6. There was approximately 0.25 tons involved.
