Information has been submitted to the Solid Waste Section-Permitting Branch and is under review. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. The facility is permitted to operate a Scrap Tire Monofill as defined in 15A NCAC 13B .1101 for the exclusive disposal of scrap tires. 2. The service area for the facility includes North Carolina, South Carolina, Virginia, West Virginia, Georgia, Tennessee and Florida. 3. The facility is secured by a gate at the facility entrance. A security guard is also present during the evening and nighttime hours. 4. Proper signage was observed at the entrance of the facility. 5. Access roads are well maintained. Operations Inspection of the Tire Monofill: 6. Shredded tires from the scrap tire processing facility and tires too large to process (off-road vehicles and heavy earth moving equipment) are cut up and disposed of in the monofill. 7. The working face kept to a manageable size. 8. No unacceptable waste was observed. 9. The facility has been approved by the Section to cover the waste with six inches of earthen materials at least twice per week. 10. Good soil cover was observed on inactive areas. 11. Continue to seed as necessary to stabilize the site. A good vegetative cover is being maintained on inactive areas. 12. Edge of waste markers are maintained. 13. Take measures to ensure that all woody vegetation is removed from the landfill. Records Review: 14.
