Permit to Operate shall expire on August 2, 2024. Permit renewal must be submitted to the Section no later than April 2, 2024. A revised Permit to Operate Application was submitted to the Section on February 5, 2021. PURPOSE OF SITE VISIT: Technical Assistance after February 22, 2022 fire. STATUS OF PAST NOTED VIOLATIONS: A. March 11, 2019 Permit to Operate Application, Site Design (Page 4, Paragraph 1), "The West Farms Wood Produces wood material treatment and processing facility is in the east central portion of the site and occupies about 6.0 acres (see Total Property Layout & Staging Area — Drawing 2)." PARTIALLY RESOLVED: A new permit application has been submitted to Donna Wilson, Permitting Engineer, for review. This violation will be resolved once the new permit application with site plan has been reviewed and approved. OBSERVED VIOLATIONS: None. Pagel of 3 NORTH CARnLINAD_E Q�� o.n.mm.m m c.w.mmen.i a.i� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS On February 22, 2022, Section staff received notification that one of the wood waste piles had caught fire. During the inspection, Mr. Conrad stated that an ember from the wood processing equipment had landed in a wood waste pile. The attendant in the wood processing area had contacted Ryan Davis, the site manager. Mr. Davis attempted to use the on -site water truck to put the fire out, but the valve broke. Mr. Davis instructed Amber Pernell to contact the fire department. The fire department arrived on -site with a tanker truck partially full of water, as they had recently put a brush fire out. At that time, wind had picked up and the fire quickly spread. Additional fire departments were called to the scene. On -site personnel utilized equipment to remove unburned waste from the burning pile and to spread out burning waste so that the fire could be put out. Mr. Conrad also stated that over 1 million gallons of water were used to put the fire out. Upon arrival at the site, the burned waste had been spread out into a windrow that was approximately 100 feet wide and a few feet tall. Personnel were utilizing equipment to move the burned waste to the western portion of the site and stage in smaller windrows. Any "hot spots" would be spread out and doused with water so that they would not flair up. Once the waste had cooled, the burned wood waste would be processed and recycled as normal. Mr. Conrad stated that following this fire, the windrows would be broken into shorter windrows with additional fire breaks. Mr. Conrad also stated that the facility would also install a 3-inch water line from their well and install a fire hydrant near the operations area. Mr. Conrad also stated that additional fire drills would be performed with personnel. During this inspection, it was determined that a majority of the water used to put the fire out had been shed to the south. Several erosion rills, the deepest was approximately 2-feet deep, were identified in the southern portion of the operations area. Sediment had inundated the erosion and sediment control features along the southern edge of the operations area. Ms. Thompson stated to Mr. Conrad that the erosion and sediment control features would need to be cleaned out and recommended that he contact Lauren Garcia with the Division of Energy, Mineral and Land Resources to ensure that proper procedures were followed.
