Combined with 2805T-Transfer-1996 PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 4 ADDITIONAL COMMENTS: 01 - Life of Site Permit was issued 2/20/2017 as a combined permit with facility #2805T and was reviewed along with the operations plan and contingency plan during this inspection. Facility is permitted to receive waste from Dare Co. Passed by the Dare Co. Commissioners on August 02, 2004. Approved waste for this facility to receive: C&D, Inert Debris, Land Clearing and Asphalt. 02 - Facility has required signage. Locking gates and fencing for security. 03 - Interior roads are constructed of all-weather materials and well maintained. 04 - Incoming load tickets were not reviewed during this inspection. 05 - Waste screening records were reviewed during this inspection and found to be up to date. 06 - Cover records were reviewed and were found to be up to date. 07 - Working face is kept small (< ¼ acre) for better management and quicker covering. Looks good. 08 - Cover materials are stockpiled on the top of the active cell and also at the cover material stockpile area. 09 - John College screened one incoming load on the working face. The load had C&D waste only. 10 - Scrap metal pile has tires pulled out to be removed to the scrap tire trailer off-site at the Dare Co. Public Works Recycling Department in Manteo. 11 - White goods containing CFC’s were upright until CFC removed and the placed onto scrap metal pile with a red X on the units. CFC removal records were not reviewed during this inspection. 12 - Windblown trash was being picked up during this inspection. Looks good. 13 - Land clearing and Yard waste area has a pile of ground and unground debris. There was no non-conforming waste noted in the piles. 14 - Facility has 6 Certified Landfill Operations Specialist: Jonathan DeWeese, expires 11/16/2021, Deborah A. Equils, expires 6/21/2022, Thomas Tillett, expires 10/9/2022, Ashley Walston, expires 4/24/2022, Neal Dennis, expires 5/3/2024 and Doug Huff, expires 5/3/2024. 15 - Edge of waste markers present. 16 - No leachate issues noted. 17 - Need to mow the sides of the cells so they can be inspected better for potential leachate breakouts and need remove small saplings. 18 - Inspected several groundwater monitoring wells. All were locked and tagged with required information. 19 - Covid-19 recommendations were followed during this inspection. 20 - Digital pictures were taken during this inspection with some attached to this report.
