The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility is permitted to receive, consolidate, and transfer yard trash. The site receives and manages leaves
collected curbside by the City of Winston-Salem Sanitation Division. No leaf grinding or composting activities
take place at this site.
2. The city contracts with Wallace Farms, Inc. to manage operations at the facility. Leaf waste is transported to the
Wallace Farms compost facility in Davie County (permit #3004) or to the Forum 52 Large Type 1 Compost
Facility (permit # 34-23) in Rural Hall, Forsyth County, for processing and composting.
3. The facility annual report for FY 2020-21 indicated that a total of 14,958.22 tons of leaf waste were collected at
the site.
4. The facility is protected by a perimeter chain link fence, and unauthorized access is prevented by two locking
gates. The eastern end of the site is occupied by the commercial business that owns the property. The facility's
operations area is separated from the commercial business with a series of orange cones.
5. New signage had been posted at the facility to conform with the conditions of the new permit that was issued on
July 20, 2021.
6. The site appeared to be in need of maintenance. Kudzu had grown up over much of the street -side perimeter
fencing and was observed to be encroaching onto the operations area from both the southern and northern
property boundaries.
7. The entire site is paved with asphalt, with four small concrete pads surrounding the operations area.
8. Coarse gravel is used to fill low areas where the old asphalt has eroded. During this inspection, several ruts and
potholes were noted in the operations area. Please ensure that the potholes and low spots in the operations
area are filled with gravel and leveled prior to commencing operations to prevent water from ponding in
and around leaf stockpiles.
9. A white dashed line marks the edge of the required buffer at the site.
10. The site is sloped so that surface water is directed to the city's storm drains. No ponding water was observed at
the site.
11. No operations were taking place at the facility during the inspection and no facility staff were onsite. The ramp,
used to load transport trailers with leaf waste, had not yet been moved into position in the operations area, but
remained stored in the northwest corner of the site.
12. One small pile of leaves had been unloaded in the operations area. Several plastic bottles and drink cups were
observed on the ground in the operations area. Immediately collect and remove the trash from the
operations area and dispose of it properly at a facility permitted to accept it.
13. Ensure that all unacceptable trash is collected and removed from the site for proper disposal before the
end of every operating day.
