LOS Permit issued 1/7/2019 for phases 1-6a. Expires 4/4/2056. PURPOSE OF SITE VISIT: Partial Facility Compliance inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - Signs posted at the entrance have all required information as stated in the permit and this facility is adequately secured by fencing and locking gates. 02 - Interior roadway is of all-weather construction and maintained in good condition. 03 - This facility has several Certified Landfill Operations Specialist: Partial listings of operators: Justin Seth Stevenson, expires 02/26/22. Thomas E. Stevenson, expires 2/26/22 and Janice Brown, expires 4/26/22, Carl Swain, expires 11/3/24 and Milton Owens, expires 8/27/24. Several other operators are available but not listed on this report. 04 - This facility also has a Certified Manager of Landfill Operations. Brad Gardner, Solid Waste Director, MOLO, expires on 09/15/2023. 05 - Edge of waste markers are present. One on the South end wasn’t visible. Need to replace or put back up if found on the ground. 06 - Scrap tires are loaded into an open top box trailer. Be sure to managed this area by the permit conditions. 07 - Screening records were reviewed during this partial facility compliance inspection. 08 - Permit and Operations plan were reviewed during this inspection. 09 - Concrete area was kept in good order. 10 - LCID area starting to fill up. The Land Clearing (LC) pile is huge. Grinding company has been contacted but is behind due to staffing issues. Be sure to keep the required 25 ft. access around the perimeter of the LC for fire control if needed. 11 - The working face is kept in a small area for faster and easier cover placement. 12 - Cell #6A was being covered during this inspection. Cover needs a bit more attention. There were a few small areas that were in need of additional cover. 13 - Cover records were reviewed during this inspection and found to be up to date. 14 - Incoming load tickets were reviewed during this inspection. 15 - 1 tire was found on the off the side slope of the working face 16 - There was a small amount of windblown trash in need of picking up by the end of the working day. 17 - Cell # 6B is in the process of finalization for review and use. 18 - Vegetation is in need of mowing to ensure inspections can be done properly. 19 - All monitoring wells were locked. Wells inspected have had the identification tags replaced or information added. 20 - Digital pictures were taken during this inspection. None attached due to a download issue. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
