NCDEQ Division of Waste Management Waste Management Solid Waste Section ENVIRONMENTAL QUALITY Notify the Section verbally within 24 hours and submit written notification within 15 days of the occurrence. (If additional space is needed, use back of this form.) NAME OF FACILITY: Wall Recycling, LLC LC1D Landfill PERMIT # 92M-LC1D DATE AND TIME OF FIRE: July 12, 2021 @ 10:00 PM HOW WAS THE FIRE REPORTED AND BY WHOM: Fire was identified by site personal at approximately 10:00 pm on 7/12/2021. The New Hope Fire Dept. was contacted and dispatched to the facility at approximately 3:00 am on 7/13/2021. Email notification of the on going event was sent by Ray Hoffman, P.E. to DEQ on 7/13/201 at 7:56 am. LIST ACTIONS TAKEN: Site personal took immediate action to extinguish the fire once identified and to move heavy equipment from area. Soil was loaded and moved to the area in an effort to smother the flames. The New Hope Fire Department was dispatched to the facility and used a tanker to spray water on the affected area. At approximately 10:00 am on 7/13/2021 Wall Recycling trucked in an additional ADT truck which was used to haul additional soils to the area. The fire department continued to spray water on the fire until late afternoon on 7/13/2021. WHAT WAS THE CAUSE OF THE FIRE: The exact cause is unknown, but believed that conditions were favorable for spontaneous combustion to occur within the unprocessed material stockpile. DESCRIBE AREA, TYPE, AND AMOUNT OF WASTE INVOLVED: The affected area consists of approximately 2.5 acres of the LCID landfill. Materials involved in the fire were processed and unprocessed trees, limbs and land clearing debris. WHAT COULD HAVE BEEN DONE TO PREVENT THIS FIRE: Spontaneous combustion often occurs sub -surface and can be difficult to identify until the point when open flame or smoldering can be seen at the surface. Facility personal acted quickly to clear area of equipment and processed materials once flames were identified and worked together with local emergency response personal to contain and control the fire. Surface inspections will be completed monthly for active and inactive areas to identify erosion fills or other openings in the cover. DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS: ral actions such as routine visual inspections for unusual ares of settlement on slopes and working faces, ensuring ease of access to water by using equipment as a water truck, keeping soil stockpiled near the active face for quick access and maintaining easy access to the fire fighting equipment such as fire guishers at all times are recommended to prevent future incidents.
