Expires July 11, 2023. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: NA The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Andrew Hammonds and David Powell with the Section inspecting Hoke County landfills. Met with April King, representing Hoke County as Landfill Supervisor, Dan Koliba has retired and Don Russell has also left before this inspection was sent. Ms. King walked with Section staff during inspection. 2. Upon inspection, the Yard Waste (YW) area with waste was roughly 200 yards by 30 yards in a windrow. This is large amount of material and should be mulched, windrowed with fire breaks and composted according to rule. Upon permit review, it was discovered that this area had recently been increased to a total of 2 acres in size. Newest permit, Application/Ops plan, Modification letter and Sheet #3 was sent to Ms. King for review on 7/8/2021. Permit Condition No. 32 - The volume of material that will be processed and stored must be contained with the two (2) - acre wood storage/stockpile and grinding area shown on drawing Sheet No. 3 of 7 included in approved operation plan (FID 1442838). 3. Permanent Edge of Waste (EOW) markers on the four corners should be installed for the approved 2 acre YW area shown on Sheet #3. Recommend large white PVC pipe. Some type of traffic control measures should be in place to manage dumping traffic into the areas in which are desired locations for the dumping. 4. Be sure and manage according to permit and best management practices. Do not allow leachate to runoff from area designated for Yard Waste Debris Processing. Also, remember waste cannot be in water, so manage accordingly. Site should be graded to allow for positive drainage of water away from the waste storage and processing area. When this material is mulched and moved, take advantage of the time to buildup these areas and slope correctly. 5. Fire breaks should be implement on the yard trash stockpiles waiting to be mulched, and the mulched material waiting to be hauled off. The mulched material windrows need this, even more so than other to ensure fire control, should a pile spontaneously combust. 6. Permit Attachment 3, Part VII, 33 - If wood material/mulch containing yard trash is provided to the public, the permittee must submit a plan for composting the yard trash in accordance with the requirements of 15A NCAC 13B .1400, et. seq. Keep this condition in mind if County decides to change its current method FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 4 of removal of the mulched material to 401 Sand LCID for slope and bank stabilization. This needs to be updated in Operations Plan, since selling to public and keeping some onsite is the approved methods for disposal currently. 7. LCID had some uncovered waste onsite. Please cover and slope according to rule. Be sure and keep inside of EOW. 8. An attendant should be on duty at all times while the landfill/YW area is open for public use to assure compliance with operational requirements and to prevent acceptance of unauthorized wastes. It is recommended that facility staff accompany customers to the LCID or YW areas when dumping to better screen waste to ensure only acceptable waste is received. 9. Trees and shrubs were visible again in some areas of LCID. Please remove when conditions allow. The vegetation on inactive areas of the LCID should be mowed when conditions allow. 10. Reminder: Excavation, grading and fill material side slopes must not exceed a ratio of three horizontal feet to one vertical foot (3:1) unless previously approved by the Section. 11. The difficulty of operating the LCID and Yard Waste area were discussed. Issues with the need for additional equipment and staff were discussed as the main issues. 12. Ms. King sent Mr. Powell some update information via email on 7/5/2021 about implementing corrective actions discussed during inspection. She indicated that “Farmers Mulch and Rock, INC. would be being mulching that week. This was later delayed due to Hurricane Elsa impacting NC. She also provided pictures of trees/shrubs removed from around LCID with cover added over most of the exposed waste. There was some exposed waste still needing proper cover in the photo. Good job but be sure and cover all exposed waste. Permit Attachment 3, Part V, 17 - Solid waste must be covered with 1-foot of suitable soil cover at least once per month or when the active area reaches 1 acre in size, whichever occurs first, or more often when necessary to prevent the site from becoming a nuisance or to mitigate conditions associated with fire, windblown materials, vectors or excessive water infiltration. A follow up inspection will be conducted about 30 days after sent date of this inspection.
