STATUS OF PERMIT:
A Permit Approval to Construct Horizontal Expansion Phases 5 & 6 and a Permit Approval to Operate Existing Landfill
& Horizontal Expansion Phases 1 -4 was issued to Louisiana-Pacific Corporation on April 27, 2020.

PURPOSE OF SITE VISIT:

OBSERVED VIOLATIONS:
15A North Carolina Administrative Code 13B .0505(a)(1)(C) states: “Areas that will not have additional wastes
placed on them for the next 12 months, but where final termination of disposal operations has not occurred, shall be
covered with no less than one foot of compacted soil.”

During the inspection, Mr. Witherspoon stated the landfill had not received waste since April of 2020. This information
was also verified via the 2019-2020 Facility Annual Report submitted to the Solid Waste Section on July 30, 2020.

Louisiana-Pacific Corporation is in violation of 15A NCAC 13B .0505(a)(1)(C) in that a minimum of one foot of
compacted soil had not been provided within 12-months of the last placement of waste within the landfill.

To achieve compliance, all areas of exposed waste must be covered with a minimum of 12-inches of compacted soil
within 90-days of issuance this inspection report.

ADDITIONAL COMMENTS

1. The landfill is permitted to accept wood ash generated from wood-fired boilers and zinc borate sludge from the
facility’s waste water treatment plant.
2. The facility operations have changed since the last inspection. Manufacturing of hardboard siding no longer
takes place at the facility.
3. The landfill has not received waste since April of 2020.
4. The landfill was not in operation at the time of inspection.
5. Groundwater monitoring was being performed semiannually as required. Sampling results for second
semiannual sampling for 2019 (performed 8/18/19), first semiannual sampling for 2020 (performed 2/25/20), 2nd
semiannual sampling for 2020 (performed 8/19-20/20), and first semiannual sampling for 2021 (performed 2/16-
17/21) were verified.

Operations Inspection of the Landfill Unit:

6. The landfill was inactive at the time of inspection and had not received waste since April of 2020. Mr.
Witherspoon explained that Louisiana-Pacific, Corp. is considering different options for the landfill to include
the disposal of new waste streams. However, no final decisions had been made at the time of the inspection.
x Please note, Solid Waste Section approval is required for any attempt to dispose of waste streams other
than those allowed by the Permit to Operate.
7. Inactive portions of the landfill were stable with native vegetation with the exception of the eastern portion of the
landfill (Phase 3B). Please see “Observed Violations” above.
8. Multiple fire boxes equipped with hydrants and hoses were provided on top of the landfill to aid in fire control.
9. During the previous inspection, a small area of bare vegetation was observed on the west side of the top of the

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

landfill. The area was approximately 20-feet wide x 30-feet long. This condition was also observed during June
21, 2021 inspection. However, there did not appear to be any change in condition of the bare area. Please ensure
that this area is seeded and stabilized with a groundcover sufficient to restrain erosion.
10. During the previous inspection, a small slope slide was observed at the top of the south facing slope
approximately 60-feet from the riser basin. Inspection of the site on June 21, 2021 found this area had been
repaired.
11. A void within the soil was observed at the crest of the south facing slope approximately 25-feet east of fire box
#37. Please investigate this area and ensure any damage is repaired to ensure the stability of the slope.
12. Since the previous inspection, a new stormwater detention pond with a skimmer and rip rap forebay had been
installed. The basin was well maintained at the time of inspection.
13. The former stormwater detention pond had been closed out since the previous inspection. The outlet of the pond
had been plugged, and the dam had been breached to a diversion swale leading to the new detention pond.
14. Permanent edge-of-waste markers were in place around the landfill.
15. The facility was secured by a gate and access must be granted via scale house personnel.
16. All access roads were of all-weather construction and well maintained.
