The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

5. The service area for the landfill is limited to Cleveland County.
6. The landfill's hours of operation are Monday through Saturday from 8:00 a.m. to 4:00 p.m.
7. The closed unlined sanitary landfill is known as the "West Site", which stopped receiving wastes as of January
1, 1998.
8. The C&D landfill is on top of the closed, unlined MSW landfill. This operation of the C&D landfill became
effective on March 23, 2000.
9. The C&D landfill is currently accepting approximately 150-tons per day.

11. The boundary of the closed unlined MSW landfill and the active C&D landfill are conterminous. White edge -
of -waste markers are used to mark the limits of these landfill units. (Photo #2)
12. No prohibited wastes were observed during the inspection.
13. The landfill was actively receiving waste during the inspection. The active working face of the C&D landfill
was well managed, and not too large. (Photo #1)
14. Waste screening is done three times each day. Waste screening records were reviewed.
15. The slopes of the landfill have a well -established vegetative cover. (Photo #2)
16. The access roads are well maintained. (Photo #3)
17. No excessive on -site erosion of the landfill was observed.
18. No ponding of water on the landfill was observed.
19. Windblown waste is very well controlled. Windblown waste is collected and returned to the working face at
close of operations each day.

4. Landfill gas monitoring wells with proper well ID
tags. (Note that landfill gas monitoring wells have their
exterior caps painted white.)
Landfill Gas Monitoring
20. Seven landfill gas monitoring locations (MP-101 — 107; P-15 and P-16); along with landfill facility structures
comprise the approved monitoring network for the "West Site".
21. Shield Engineering conducted the most recent landfill gas monitoring event occurred on February 23, 2015.
22. Landfill gas measurements are taken with a LANDTEC GEM 5000TM landfill gas monitor.
23. No exceedances above the 5% of the LEL of methane were detected any of the seven landfill gas wells.
24. Landfill gas monitoring wells are identified by painting the caps of the well casing white. (Photo #4) (Please
ensure that all monitoring wells have the required identification plate in accordance with the well
construction standards in 15A NCAC 02C .0108(c)(11) and in accordance with Permit Condition 50, of
Part III, of Attachment 3, of the Permit to Operate issued to Cleveland County on February 28, 2011.
Water Quality Monitoring
25. The "West Site" (Figure #1) is under a Corrective Action Plan (CAP) for the use enhanced bioremediation in
conjunction with a gas intercept trench, to remediate contaminated groundwater in the southern portion of the
facility. This CAP was approved on October 54, 2007.
26. The most recent water quality monitoring event was conducted by Shield Engineering, Inc. on April 8, 2015.
27. Six groundwater monitoring wells (MW-8, 9R, 10, 11, 12 and MW-13) comprise the approved groundwater
monitoring network for the "West Site". MW-8 it the background (i.e. up -gradient) groundwater monitoring
well.

White Goods & Scrap Metal Area
29. White goods and scrap metal are collected from the public at each of the County's nine convenience centers.
Cleveland County then hauls the white goods and scrap metal to a staging area located NW of the two
leachate above ground holding tanks. (Figure #1)
30. White goods and scrap metals are staged on a concrete pad. This area is well organized. (Photo #5)
31. Contracts for the removal of White goods and scrap metals are issued to the highest bidder. Morris Scrap
Metal in Kings Mountain, NC won the most recent contract for the removal of these items.
32. CFCs (refrigeration gases) are removed from the white goods by Rapid Recovery of Pineville, NC.
Electronics Recyclin
33. Electronics recycling is done by Cleveland Vocation Industries in Shelby, NC.
34. Electronics are collected and staged at the counties nine "convenience centers".

Scrap Tires
29. Scrap tires are collected and stored in semi -trailers next to the scale house for the C&D landfill prior to being
shipped to US Tire of Concord, NC. (Photo #6)
30. The scrap tire area is well managed.
Treatment and Processing Area
31. Wooden pallets are collected and staged north of the "East Site" closed MSW landfill. This material is
ground into boiler fuel by a subcontractor. (Photo #7) Contracts for the grinding of the wooden pallets are
issued to the highest bidder.
32. Yard waste is ground into mulch for use on site and for sale to local business. Deveny's out of Casar, NC is
under a five year contract to conduct the grinding (Photo #8). Contracts for the grinding of yard wastes are
issued to the highest bidder.
33. The leaf composting area is SW of the barrow area. The leaves are passively composted for use as a soil
amendment on site. (Photo #9)
Leachate Manageme
29. The leachate from the Self-McNeilly MSW landfill is pumped via a force main to two 348,000- gallon above
ground leachate holding tanks located northwest of the closed "East" MSW landfill. (Photo #10) The
leachate is automatically discharged from the holding tanks directly to the City of Shelby Waste Water
Treatment Plant for disposal.
30. All of the storm -water sediment basins were well maintained and had ample freeboard. All of the sediment
basins are scheduled to be cleaned out before the end of June 2015.
