Box 729 Burnsville, NC 28714 PARTICIPANTS: Tim Parker, Parker Lumber Deb Aja — Solid Waste Section STATUS OF PERMIT: Active. The YWN notification was received on May 9, 2013 and reviewed on May 21, 2013. The facility may continue to operate by notifying the Solid Waste Section on an annual basis by June 1st of each year. PURPOSE OF SITE VISIT: Comprehensive Inspection. STATUS OF PAST NOTED VIOLATIONS: N/A. OBSERVED VIOLATIONS: There were no violations observed. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITONAL COMMENTS 1. The site entrance was controlled by a locked gate. 2. The facility is allowed to receive land clearing waste [stumps, tree trunks, limbs]. The facility accepted only those that it is allowed to receive. 3. The wood waste is ground on -site, accept that stumps are delivered to a permitted facility for grinding. 4. There appeared to be less than a total of 6000 cubic yards of unprocessed and processed wood waste on site at the time of the inspection. 5. The facility operational area appeared to be less than two acres in size. 6. There was positive drainage away from the processing and storage area. No surface or standing water was observed around processed and unprocessed wood waste. 7. There was no leachate observed around the processed and unprocessed wood waste. 8. There was no evidence of erosion or movement of silt or contaminants from the site. 9. The access road was maintained. 10. This site has been approved as a temporary disaster debris staging site (DS100-001). Please contact me if you have any questions or concerns regarding this inspection report.
