STATUS OF PAST NOTED VIOLATIONS:
1. A Notice of Violation (NOV) was issued on May 21, 2013 for 15A NCAC 13B .0201(a) which states:
No person shall treat, process, store, or dispose of solid waste or arrange for the treatment, processing, storage
or disposal of solid waste except at a solid waste management facility permitted by the Division for such
activity, except as provided in G.S. 130A-294(b).
The facility has been issued a new Yard Waste Notification number since the previous T&PN number had
expired. With the new Notification (YWN-50-008), the NOV is now resolved.

OBSERVED VIOLATIONS:
No new violations were observed.
ADDITIONAL COMMENTS
1. All Photographs were taken by Troy Harrison on September 6, 2013
2. The facility is in the process of removing all of the material that is remaining at the facility. There was a lot
of dirt mixed with woodwaste. According to Mr. Heinlein and Mr. Carmichael a separator is going to be used
to remove the dirt from the woodwaste which will then be ground. Any ground material that remains to be
used as ground cover must be approved by the Solid Waste Section and Land Quality Section.
