NAME OF FACIJATY: Gaston County Landfill PERMIT 9 36-06 DATE AND TIME OF FIRE: 04-19-2021 HOW WAS THE FIRE REPORTED AND BY WHOM: @ 8:00 am The fire was first seen by Michael Deese, Landfill Operator. He immediately called fire Landfill Administrator, Barry Cloninger to bring the water truck. LIST ACTIONS TAKEN: Dnce Barry was called, he called several other operators to start moving dirt to bring to the area and the second water truck. The operators were able to build some iirt berms up around the area while applying water. There was a valve issue with one of the water trucks, so staff called the fire department out of an abundance of :action. Once the fire was out, staff continued moving the affected waste around while spraying with water. Operators also continued stockpiling dirt near the area. Fire department staff stayed to help determine the cause of the fire. WHAT WAS THE CAUSE OF THE FIRE: It is not immediately clear what started the fire. There were no hot spots on the working face as we started accepting waste this moming. A truck had just dumped, and the operator was pushing out the load when the first flames were seen. After the fire was put out, fire department stayed and helped staff look for the cause of :he fire. While it was clear that it started in a certain section of waste, there was no particular piece that was clear to have started the issue. DESCRIBE AREA, TYPE, AND AMOUNT OF WASTE INVOLVED: This happened on the working face of Unit H (M8W). The initial flames were part of one load, but in the actions of spreading it out and covering with water, the entire area covered about 40#Ix2011. WHAT COULD HAVE BEEN DONE TO PREVENT THIS FIRE: We have told the scale house staff to make sure to look into the containers when trucks are weighing in, and the landfill operators to be very alert when trucks are Jumping to be on the lookout for hot loads. We always keep a water truck full onsite, which was critical in being able to get the fire put out as quickly as possible This morning. We will continue that practice moving forward. DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS: All staff is trained is waste screening activities. That is an annual requirement. We will continue to stress screening activities moving forward. We also always .keep a full water truck at our site to utilize in these events. This will be continued. Staffalso participates annually in fire fighting training to know what to do in this scenario. That training will continue moving forward. Also, every piece of equipment and each vehicle on our site are equipped -with fire extinguishers.
