with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1. All photos included in this report were taken by Susan Heim during the inspection on January 13, 2021, unless
otherwise noted.
2. This inspection was conducted as a pre -operation inspection to ensure the facility's compliance prior to
beginning to accept waste in Phase 2 of the newly permitted 6.479-acre site, and to begin operating a small type
1 compost facility and treatment and processing units for wood waste and used concrete.
3. The facility is permitted to accept only land clearing debris, untreated wood, yard trash and inert debris that is
strictly limited to untreated/unpainted concrete, brick, concrete block, uncontaminated soil, rock, and gravel.
4. The facility appeared to be clean and no windblown litter was observed.
5. Unauthorized entry into the facility is prevented by means of fencing, wood lines and berms, and the facility is
secured by means of a locking gate at the entrance. Required signage was observed at the facility entrance and
along the access roadway.
6. The entrance roadway is of all-weather construction, made of compacted stone, and was found to be in good
condition. An access roadway leading from the main parking area up the south side of the landfill to the tipping
areas appeared to be stable and in good condition, capable of accommodating landfill traffic and equipment.
7. Landfill operations are conducted from an office located at the end of the entrance drive. Incoming waste is
tracked by volume. Records are maintained in the office and are available for review upon request.
8. The facility was operating during the inspection, and customers were observed dumping loads of brush and tree
waste. Landfill staff met and directed each customer to a specific unloading spot, then screened the waste as it
was unloaded. A large horizontal shredder was staged in the wood waste storage area, and a track hoe was
observed loading large logs onto a log truck.
9. An office is located at the end of the entry drive east of the landfill. All facility records and plans are maintained
in the office. An operator is on duty during operating hours.
10. The landfill has an access roadway that runs along the southern and eastern perimeter of the fill area that was
observed to be of all-weather construction and made up of compacted stone, as was the entry roadway. A
narrow dirt road runs along the northern portion of the site and provides access to the sediment pond so that
maintenance can be performed regularly as required. The northern area of the site, outside of the phased landfill
construction, was not observed during this inspection. However, Mr. Boyles stated that the northern road is
muddy but navigable and that the pond was cleaned out recently to ensure adequate capacity for additional rain
events, as recommended by the Winston-Salem erosion control inspector.
11. The sediment pond was observed from the Phase 2 excavated area to the north of the existing landfill area. The
pond was almost filled with water, but the baffles appeared to be intact. Reports from the Winston-Salem
erosion control inspector confirmed that all erosion control measures had been properly installed and were
functional.
12. Three phases of landfill development are included in the permit to construct. The first phase involves the
notified 2-acre fill area. Phases 2 and 3 are lateral expansions north of the original landfill, and Phase 4 is a
vertical expansion that will tie in Phases 2 and 3 to the original notified landfill area.
13. Edge of waste markers had been installed to identify the perimeter of the entire landfill as well as the
boundaries between Phases 1 and 2, and between Phases 2 and 3. Markers were spaced so that the perimeter of
the fill area is easily discernable along each side of the landfill. Mr. Boyles noted that a previous error in
placement of the markers had been corrected. During this inspection, the original 2-acre disposal area appeared
to be well within the marked boundary.
14. As -built drawings were included in the final permit application for the Phase 2 construction/excavation. Phase 2
is a L-shaped area that surrounds the western and northern limits of the original Phase 1 notified landfill site.
Excavated slopes were observed to be of a grade sufficient to prevent erosion and generally in good condition.
Due to the recent heavy rain events in the area, some small rills were beginning to develop on the excavated
slopes. However, the eastern interior slope of Phase 2 had been repaired and some vegetative cover was evident
in this area. Verbal permission was given during the inspection to begin accepting waste in Phase 2.
15. Excavation in Phase 3 was still underway at the time of the inspection. Please provide as -built drawings and a
construction certification to Permit Engineer Ming-Tai Chao (ming.chao(ancdenr.gov) when
construction of Phase 3 is completed so that an inspection can be scheduled prior to accepting waste in

16. The southern slope of the original fill area had been covered with clean soil, graded, and seeded, and a
vegetative cover was becoming established. Some signs of erosion were visible along the slope. Perform
maintenance to repair the erosion rills evident on the southern side slope to preserve the existing
vegetative cover and to prevent waste from becoming exposed by future rain events.
17. The eastern slope of the original landfill showed signs of erosion, and some of the waste had become exposed as
a result. Ensure that areas of exposed waste on the eastern slope of the landfill are compacted, adequately
covered with clean soil, and re -seeded to stabilize the slope and prevent erosion. (See comment 21 for
additional information.)
18. The western slope of the landfill was covered in most areas, and some vegetation was evident. However, the
northwest corner of the side slope appeared to be steep and unstable, consisting of exposed concrete and rock.
Chunks of concrete and rock were observed in the excavated area of Phase 2 that appeared to have fallen from
the northwest corner of the side slope. Ensure that the northwest corner of Phase 1 is regraded, compacted,
and covered as required. (See comment 21 for additional information)
19. The high, steep northern slope of the landfill was found to be only partially covered with soil. Some large pieces
of inert materials and wood waste were visible in several areas, as was the case during the previous inspection
on August 26, 2020. Mr. Boyles stated that the wet weather has made it unsafe to move equipment onto the
slope to apply cover soil, and that the soil that had been excavated from Phase 2 for that purpose was too muddy
to spread effectively. If the slope is too steep to allow equipment to perform maintenance and repairs
safely, it is recommended that the waste be pulled back so that the degree of slope is reduced sufficiently
to allow required operations to be performed on a regular basis. (See comment 21 for additional

20. The mulch piled in the northeast corner of the Phase 1 disposal area remained in place, as noted during the prior
inspection on August 26, 2020. Mr. Boyles had previously expressed his intent to move the mulch to the new
wood waste processing/compost areas rather than disposing of it in the landfill. (See comment 21 for additional
information.)
21. It is required that all waste shall be covered with no less than 6 inches of soil monthly, or when the
working face reaches one acre in size, whichever occurs first. To ensure compliance with 15A NCAC 13B
.0566(3), all waste in the landfill shall be covered within 30 days of the date of this inspection. Any waste
not intended to be landfilled will be removed and disposed of or stored properly.
22. A cover log shall be established that includes the date that monthly cover is applied to all of the waste in the
landfill. The log may also include the amount of soil used to achieve the minimum 6" cover and the man hours
spent, if desired by the landfill operator. The cover log shall be maintained in the landfill office and made
available for review upon request.
23. The landfill has been constructed in a series of 6' — 8' lifts that have spanned the Phase 1 disposal area. The
tipping area is on the top of the landfill and appeared to be constructed of compacted soil and mulch, providing
a stable surface for vehicles and equipment to maneuver. A number of small piles of wood waste were visible
around the tipping area where customers had dumped their loads. Mr. Boyles explained that the waste is
generally pushed onto the landfill slopes at least daily. Although the tipping area was wet and muddy, no major
equipment ruts or ponding water were observed on the landfill top.
24. A berm had been constructed south of the landfill to create the 0.38-acre area designated on the approved plans
to be used for wood waste recycling. Brush was stockpiled in this recycling area, located between the access to
the tipping area and the berm. Wood waste piles appeared to be in excess of the maximum allowed by the
permit. Ensure that wood waste stockpiles are constructed to be no taller than 12', as mandated in
Attachment 3, Part VII, permit condition 5.b.v. and on plan sheet C-1 of the approved final permit
application.
25. The treatment/processing and compost areas had not yet been constructed on the site at the time of this
inspection. No inert debris stockpile was observed. The inert debris operations area will be located on the east
side of the landfill, will occupy approximately'/4 acre, and will be constructed of crushed stone and concrete.
Mr. Boyles pointed out the area where the mulch and compost operations will take place, south of the wood
waste stockpile and berm. He stated that a separate %2 acre pad of compacted clay and soil would be constructed
for each. Please contact the Solid Waste Section to schedule an inspection of these 3 treatment and

processing areas after construction is complete and prior to beginning treatment and processing
operations.
