Issued January 24, 2017. Life of Site Permit. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - This permit issued on January 24, 2017, is a Life of Site Permit as defined in N.C.G.S. 130A-294(a2). Copies of this permit, the approved plans, and all records required to be maintained by the permittee must be maintained at the facility and made available to the Section upon request during normal business hours. 02 - The facility is permitted to receive solid waste generated within Chowan County, North Carolina. Waste transported for disposal shall be sent to the East Carolina Regional Landfill in Aulander (Permit No. 08-03). 03 - Interior road is constructed of all-weather material and maintained well. 04 - Windblown trash is picked up multiple times during the day and checked at the end of the workday. During this inspection, a small amount of older windblown trash was noted and needs to be picked up today. 05 - The facility is secured by locking gates and fencing. 06 - Facility has signs posted with required information. 07 - Leachate appears to be contained at all times as it is draining into the sewer drain. 08 - Tipping floor and load out bay are clean during this inspection conducted during working hours. 09 - This facility uses a compactor and the waste is loaded thru the rear of transport trailers. Facility is currently working with DWM permitting to do a modification to allow adding a loading pad to ensure the trailers are full. Compactor does not fill trailer full. This was noted on the last inspection report. Process may be delayed further. 10 – Due to COVID-19, no records were reviewed during this inspection. 11 - This facility has 3 Certified Transfer Station Operations Specialist: Clarence Radkins, expires 5/31/22, Christopher McNeil, expires 5/31/22 and David Myers, expires 10/9/22. 12 - COVID-19 recommendations were followed during this inspection. 13 - Noted on last inspection report; Facility has some concrete stored at facility for grinding. If 75 % is not ground within this calendar year, facility may have to get notified LCID for storage of this type of solid waste. This waste has since been removed. 14 - Digital pictures taken during this inspection. Several attached to this report.
