A copy of the comments resulting from this review is attached for your reference. 'A revised Permit Application and Operation & Maintenance Manual that incorporate responses to these comments should be submitted for our review and approval as soon as possible. Where appropriate, add or revise narrative in the text of both the Permit Application and the Operation & Maintenance Manual that addresses the issues discussed in the comments. I. General 1. Within the operation manual on pg 5 the vector control standard shall be 14 days over 104 F with an average temperature, over 113F. 2. Within the operation manual on pg 10 the fecal coliform level shall be less than 1,000 mpn/g. 3. The operational hours of the facility need to also be listed. II. Section 2.0 Application Requirements 1. 15A NCAC 13B .1405 (a) (1) stipulates that an aerial photograph or scaled drawing is needed, where one inch is less than or equal to 400 feet, accurately showing the area within one-fourth of the mile of the proposed site's boundaries with the following specifically identified: (A) Entire property owned or leased by the person proposing the site; (B) Location of all homes, wells, industrial buildings, public or private utilities and ati roads, watercourses, dry runs, and other applicable informon regarding the general topography within one-fourth mile. An aerial photograph or detailed drawing needs to be submitted to fulfill the requirements as listed above. 2. 15A NCAC 13B .1405(a)(2) A letter from the unit of government having zoning jurisdiction over the site which states that the proposed use is allowed within the existing zoning, if any, and that any necessary zoning approval or permit has been obtained. 3. 15A NCAC 13B .I405(a)(3) An explanation of how the site complies with siting and design standards in Rule .1404 of this Section (specifically): >, to detailed narrative should address how the site was previously permitted in the 100 yr floodplain and within 50' of a stream. 4. 15A NCAC 13B.1405(a)(5) Site plans at a scale where one inch is less than or equal to 100 feet to the inch that delineates the following: (A) Existing and proposed contours, at intervals appropriate to the topography; (B) Location and elevations of dikes, trenches, and other water control devices and structures for the diversion and controlled removal of surface water; (C) Designed setbacks and property lines; (E) Areas for unloading, processing, active composting, curing, and storing of material. 5. 15A NCAC 13B .1405(a)(6) A description of the operation of the facility, which must include at a miriimum: (B) List of personnel required and the responsibilities of each position. 3 (R) Moo" h ency plan for disposal or alternative usage of residues or finished compost that cannot be used in the expected manner due to poor quality or change in market conditions. 6. 15A NCAC 13B.1405(a)(7) A report on the design of the facility, including: (A) Design capacity of the facility (B) A process flow diagram of the entire facility, including the type, size, and location of all major equipment, and feed stock flow streams. The flow streams shall indicate the quantity of material on a wet weight and volumetric basis; 7. 15A NCAC 13B. I 405(a)(8) A description of the label or other information source that meets the requirements of Rule ,1407(g) of this Section. 8. 15A NCAC 13B.1405(a)(9) Plans and specifications for the facility, including manufacturer's performance data for all equipment selected. 9, 15A NCAC 13B.1405(a)(10) A detailed operation and maintenance manual outlining (specifically): (C) An outline of reports to be submitted in compliance with this section 2 10. 15A NCAC 13B.1405(a)(11) As -built drawings where applicable End of comments
