Permit renewal application is currently under review. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. All photos included in this report were taken by Susan Heim on October 30, 2020 during this inspection. 2. This facility serves as the yard waste treatment and processing site for Forsyth County. Composting is no longer a permitted activity at this facility. 3. The facility is permitted to receive wood waste, including: yard waste; unpainted and untreated wood waste and wood pallets; woody waste from disaster debris management and land clearing activities; and, leaves and grass clippings. 4. The City of Winston-Salem contracts with Wallace Farms, Inc. to manage operations at the facility. Wallace Farms provides a site supervisor who serves as the facility's operations manager and is on duty during operating hours. A City of Winston-Salem staff member serves as the scale house operator. 5. Grinding operations take place onsite, and all processed wood waste is loaded into trailers and transported to the Wallace Farms facility in Davie County (permit # 30-04) for further processing and composting. 6. Copies of the facility Permit to Operate and the approved Operations Plan are kept on file in the scale house and are available for review upon request. 7. Tonnage records of waste received at the facility were provided via email following the inspection. For the previous fiscal year, materials received at the facility totaled 24,767.17 tons. Monthly tonnage for the current fiscal year was recorded as follows: July 2020 — 2,895.60 tons; August 2020 — 2,299.21 tons; and, September 2020 — 3,511.76 tons. 8. The site is surrounded by a chain link fence, and unauthorized access is prevented by two locking gates. One gate is located in the northwest corner of the site and serves as the facility entrance; the other is located in the southeast corner of the site and is used, when needed, for tractor trailer access. 9. Proper signage was observed at the entry gate, and included emergency contact information, operating hours, permit number and acceptable wastes. Inside the facility, directional signs are strategically placed to assist customers in finding the correct location for unloading specific materials. 10. The facility was observed to be clean and well -maintained. 11. All incoming and outgoing loads are weighed and recorded. The scale house operator uses a closed-circuit camera system to screen incoming loads prior to directing them to the proper tipping location. 12. Operations are divided into two sections. The northern half of the site is used to accept and stockpile large truck loads of incoming wood waste, and the southern half includes an area dedicated to the dumping of small loads brought in by residents. This site plan effectively separates residential and commercial traffic at the site and promotes the safe movement of vehicles and equipment. 13. The entire site is paved, and access roadways were found to be in good condition. Areas of broken and missing pavement were noted in the operations areas, and water had ponded in several of these. However, no ponding water was observed in or around the piles of wood waste. Mr. Cain stated that a contract was currently being awarded for the repaving of the operations areas to ensure an impervious surface. 14. A roll -off container is staged on the north side of the site and is used to collect unacceptable waste that is separated from incoming loads. The container is taken to Hanes Mill Landfill (Permit # 3402) as necessary to properly dispose of the waste. 15. A large tub grinder was staged in the middle of the main operations area; however, no grinding was taking place during the inspection. 16. Horizontal grinders were observed on the west side of the site, and one is permanently installed, with safety railing protecting the walkways and a fire extinguisher mounted near the control panel. 17. An above ground fuel storage tank is installed in the southwest corner of the facility. This area is also used to store equipment and parts. 18. Stockpiles of processed wood waste were observed onsite and appeared to meet the size restrictions of 30' high and 50' wide. 19. Storm water disperses and sheet flows across the site. An asphalt berm, situated near the top of the driveway leading to the back (southeastern) gate, slows the flow of surface water and directs it to the grassy area along the southern property line. No evidence of erosion or sedimentation was visible during this inspection. 20. The facility appeared to be in compliance with required setbacks. Uncontaminated wood pallets are separated from other wood waste and stockpiled near the southeastern corner of the property. 22. No unacceptable waste was observed at the facility.
