Issued; 5/31/2019 and expires; June 09, 2058. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - This Facility has several Locking gates and fencing for security. 02 - Signage is posted and has required information. 03 - Roads are constructed off all-weather materials and are maintained. 04 - Facility is permitted to receive waste from Edgecombe, Nash, Halifax, Wilson, Martin and Pitt Counties. 05 - The Waste from this Transfer Station is permitted to be taken to The East Carolina Regional Landfill (08-03) and/or to Sampson County Disposal, LLC (82-02). 06 - This facility has 4 Certified Transfer Station Operations Specialist: Larry Moore: expires 05/25/22, Travis D. Hudson: expires 12/08/21, Gloria A. Mosely: expires 10/20/23 and James Hucks, expires 05/25/22. 07 - Leachate tanks are emptied bi-monthly. Records of removal were not reviewed. 08 - The windblown trash issues from the past has improved and in compliance today. 09 - Screening records are being kept at the tipping floor breakroom and were reviewed and up to date. 10 - Tipping floor was clean as can be during working hours. 11 - The lower I-Beams behind the 2 push walls had minimal dust today. Be sure to wash them while washing the floor. 12 - Minimal waste on the tarmac as well today during this inspection. 13 - No leachate leaking from stored trailers today. 14 - Be sure to tarp trailers as soon as possible to keep buzzards from pulling out trash or windblown trash.Observation. 15 - Overall management has improved. 16 - Digital pictures were taken during this inspection and several are attached to this report. Suggestion only. None observed and Tarmac with minimal trash.
