Charlotte, NC 28216 PARTICIPANTS: all Smith, Mecklenburg County, LUESA Wayne Kerr, Piedmont Grading, Owner STATUS OF PERMIT: Demolition Landfill permit 60-Q issued 6/17/1985 Permit to Operate reissued as 60-Q-LCID 7/09/1999 Permit to Operate renewed 3/26/2018 Permit Expiration date: 3/03/2023 PURPOSE OF SITE VISIT: Operational Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: No violations observed Date of Last Inspection: November 85 2019 The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 3 FACILITY COMPLIANCE INSPECTION REPORT Mecklenburg County Land Use and Environmental Services Agency Solid Waste Division ADDITIONAL COMMENTS: 1. The facility is permitted to accept land clearing waste, yard waste, untreated and unpainted wood, uncontaminated soil, and inert debris such as rock, brick, concrete and concrete block. No unapproved waste was observed during inspection. 2. Wayne Kerr stated that currently, they mainly receive brush, stumps and rock. The facility receives an estimated 50 loads per day. 3. Proper signage with permit number and emergency contact information was observed at the entrance. 4. The facility entrance is secured by a gate which can be locked during non -business hours. 5. Incoming loads are screened by an overhead camera and a gatehouse attendant prior to driving to the active face. 6. Entrance and interior roads were well maintained. No standing water was observed in roads or in contact with waste. 7. Water appears to be properly diverted towards a sediment basin at the toe of the southeastern slope of the landfill. 8. Western side slopes appear to be in compliance with the permitted 3:1 ratio. Observed established vegetation on western side slope. The northeast and eastern slopes are currently being regraded to achieve more gentle slopes. Previously missing edge of waste markers have been installed along the western side of the landfill. 10. The active working face is kept to a small area less than the permitted one acre. It appears that adequate soil cover is being applied to closed cells. Observed an adequate supply of soil for cover material or in case of fire. 12. No fugitive dust or windblown materials were observed.
