Bunnell-Lammons Engineering, Inc. (BLE) has monitored and documented the Cells No. 3B, 4, and 5 construction tasks listed below to determine if the construction was performed in accordance with the project drawings, specifications, and conditions, as approved by the North Carolina Department of Environmental Quality (NC DEQ), Division of Waste Management. The report will be presented in three volumes as outlined below. The landfill is owned by Waste Management of Carolinas, Inc. Cells 3B, 4, and 5 occupy approximately 12.7 acres (553,214 sq. ft.) north of the existing Cells 1B and 2 at the landfill. The majority of the earthwork performed to reach the design subgrade elevations in the cells consisted of mass excavation. Mass excavation included the removal of partially weathered rock and blasting of non-rippable rock. Structural fill was placed to bring the cell floor to the design grades after the blasting was completed. The Cells 3B, 4, & 5 base liner will consist of a subgrade (base grade), overlain by the secondary textured 60-mil geomembrane. The secondary geomembrane will be overlain by a leak detection system (LDS), composed of an LDS geocomposite having a minimum transmissivity (Θ) of 5.1 x 10-9 m2/s, and approximately 1,970 lf of perforated 8-inch diameter, SDR 11 HDPE pipe, surrounded by ASTM No. 57 stone. Approximately 160 lf of solid wall 8-inch diameter, SDR 11 HDPE pipes serve as the clean-out accesses for the LDS perforated pipes. The clean-outs are located on the south and east sides of the cells. The leak detection system will be overlain by a geosynthetic clay liner (GCL). The GCL will be overlain by the primary textured 60-mil HDPE geomembrane. The primary geomembrane will be overlain by a leachate collection system (LCS), composed of an LCS geocomposite having a minimum transmissivity (Θ) of 4.2 x 10-4 m2/s and approximately 1,970 lf of perforated 8-inch diameter, SDR 9, HDPE pipe, surrounded by ASTM No. 57 stone. It also includes approximately 400 lf of solid wall 8-inch diameter, SDR 11, HDPE pipes which serve as the clean-out accesses for the perforated pipes. The clean-outs are located on the north and west sides of the cells. The protective cover consists of a minimum 2-foot thick layer of screened material with a maximum particle size less than 3-inches (approximately 41,830 cy). The protective cover material was obtained during mass excavation in the proposed cell footprint. The ASTM No. 57 stone was obtained from an off-site quarry. The leachate system drains by gravity to the northeast end of the cells, where it ties into the existing Cell 3A leachate collection system. The purpose of this document is to report construction observations and testing by BLE and provide the CQA report required by Title 15A, NCAC 13B, Section .1624 of the North Carolina Department of Environmental Management (NCDEQ) and as outlined in the project contract documents and the CQA Plan. Volume 1 of 3 of this report documents the following construction of Cells 3B, 4, and 5: • Structural fill and subgrade preparation. Volumes 2 and 3 will document the following construction tasks of Cells 3B, 4, and 5: • Secondary 60-mil HDPE Geomembrane (Flexible Membrane Liner, FML) • Leachate Detection System (LDS) Geocomposite • Reinforced Geosynthetic Clay Liner (GCL) • Primary 60-mil HDPE Geomembrane (Flexible Membrane Liner, FML) • Leachate Collection System (LCS) Geocomposite Construction of Cells 3B, 4, and 5 June 22, 2020 Great Oak Landfill BLE Project No. J20-13334-02 Randolph County, North Carolina 1-2 • Protective cover layer, and • Leachate collection system. 2.0 SUMMARY OF QUALITY CONTROL/QUALITY ASSURANCE PARTIES The following parties were responsible for various aspects of quality control/quality assurance during the construction of this project. 2.1 WASTE MANAGEMENT OF CAROLINAS, INC. Waste Management of Carolinas, Inc. (Waste Management) is the owner and operator of the Great Oak Landfill. 2.2 PROMUS ENGINEERING, LLC Promus Engineering, LLC (Promus) of Chattanooga, Tennessee, is the Design Engineer for the Great Oak Landfill. Promus prepared the project construction plans and a portion of the specifications. S&ME of Raleigh, NC prepared the original permitted technical specifications and CQA Plan. SCS Engineers of Charlotte, NC revised the technical specifications and CQA Plan in 2017. 2.3 EARNHARDT GRADING, INC. Earnhardt Grading, Inc. (Earnhardt) of Stanley, North Carolina, was the general contractor for the construction of Cells No. 3B, 4, and 5. Earnhardt Grading will also install the protective cover and leachate collection system. 2.4 ENVIRONMENTAL SPECIALTIES INTERNATIONAL, INC. Environmental Specialties International, Inc. (ESI) was contracted by Waste Management to complete the geosynthetic installations of the 60-mil minimum textured HDPE geomembrane, the LDS and LCS geocomposite, and the geosynthetic clay liner (GCL) for Cells No. 3B, 4, and 5. The geomembrane was manufactured by Agru America, Inc. of Georgetown, South Carolina. The the LDS and LCS geocomposite was manufactured by SKAPS Industries of Commerce, Georgia. The geosynthetic clay liner was manufactured by Solmax of Houston, Texas. 2.5 SURVEY CAROLINA PLLC Survey Carolina PLLC (Survey Carolina) were retained by Earnhardt to serve as the surveyor of record. Survey Carolina performed the as-built surveys to document the rock blasting, subgrade and the top of the protective cover and the leachate collection system. 2.6 BUNNELL-LAMMONS ENGINEERING, INC. Bunnell-Lammons Engineering, Inc. (BLE) was retained by of Waste Management to serve as the independent third-party Construction Quality Assurance (CQA) firm. BLE has observed, tested, and documented the procedures, processes, results, and related work, used to install, or construct the various landfill components. BLE has specifically provided quality assurance field and laboratory testing during the construction. The fieldwork has been performed under the direction of the CQA engineers, Mr. Jonathan Usher, P.E., and Mr. Bjorn Sundquist, P.E. 3.0 CONSTRUCTION OBSERVATION AND TESTING Construction observations are contained in the daily reports, which are presented in Appendix B, and a series of color photographs of major project features is included in Appendix C. The general construction Construction of Cells 3B, 4, and 5 June 22, 2020 Great Oak Landfill BLE Project No. J20-13334-02 Randolph County, North Carolina 1-3 procedures observed by BLE and the testing performed during landfill construction are summarized in the following report sections. 3.1 CELLS 3B, 4, and 5 3.1.1 Site Preparation Prior to construction, site vegetation and topsoil were cleared and grubbed to allow for the removal of overburden soils and rock. Site grading to achieve the design subgrade elevations required mass excavation and placement of structural fill. The excavation was performed by dozers and track excavators loading articulated off-road trucks. Overburden soils and partially weathered rock above the design subgrade elevations were ripped and removed from the Cells 3B, 4, and 5 area. Earnhardt used blasting to remove rock that could not be excavated using conventional construction equipment to a minimum of 4 feet of separation between the top of the subgrade and the top of bedrock. The soils and blasted rock excavated during Cells 3B, 4, and 5 mass excavation were stockpiled in the on-site borrow area to the west of Cells 3B, 4, and 5 for potential use as structural fill or protective cover. The stockpiled soils were screened through a 3-inch mechanical screen for use as structural fill and protective cover. 3.1.2 Subgrade Preparation During the Cells 3B, 4, and 5 construction, earthwork grading to obtain the design subgrade elevations within the cells required earthwork cuts in a majority of the cell to have a minimum 4-foot buffer between rock and the subgrade. Based on the survey data provided by Survey Carolina, PLLC and our site observations, the minimum 4-foot buffer was verified by a BLE Professional Geologist, and these reports are presented in Appendix D. Areas where structural fill was placed were successfully proofrolled using a loaded off-road haul truck. The proofrolling was monitored by the CQA engineering technician under the direction of the CQA engineer. Structural fill soils were placed in maximum 8-inch thick loose lifts to reach the design subgrade elevations. The structural fill soils placed in the cell consisted primarily of sandy silt and sandy clay borrowed from earthwork cut areas within the cell limits. The surface of the soil subgrade was compacted, and in-place density tests performed to confirm that compaction requirements were achieved. The Cells 3B, 4, and 5 Base Grade As-Built Survey, prepared by the project surveyor (Survey Carolina) is presented in Appendix A of this report volume. The CQA technician performed field (in-place) density tests on the compacted structural fill and subgrade in Cells 3B, 4, and 5, using the nuclear gauge method (ASTM D6938). The field sketch used for referencing density test locations is provided in Appendix A (Figure 2). The test locations are referenced to a series of grids with an area less than or equal to 10,000 sf to assist in achieving the specified in-place density test coverage. The in-place density tests were compared to laboratory standard Proctor compaction tests (ASTM D 698) of the site soils. Standard Proctor compaction test results representative of the structural fill soils are presented in Appendix E. The results of the field density tests are presented in Appendix F, and document that the structural fill was placed in accordance with the project Contract Documents and CQA Plan. In conclusion, it is our professional opinion the structural fill and subgrade for Cells 3B, 4, and 5 have been constructed in accordance with the following: • The Construction Plans and Contract Documents, • The CQA Plan, Construction of Cells 3B, 4, and 5 June 22, 2020 Great Oak Landfill BLE Project No. J20-13334-02 Randolph County, North Carolina 1-4 • The requirements of the NCDEQ Solid Waste Disposal Permit, • Acceptable Engineering Practices, and the soil subgrade is acceptable for placement of the project geosynthetics. This statement is based on the observation, field testing, and laboratory testing by CQA testing personnel under the direction of CQA Engineer for Bunnell-Lammons Engineering, Inc. and the provided as-built survey information by Survey Carolina. Prior to deployment of the geosynthetics, each section of the soil subgrade will be approved by the CQA technician based on guidance for acceptable surface preparation provided by the CQA engineer. N/A Upon arrival, rep conducted a geologic observation of the exposed shot rock located near grids 12-13. Area of shot rock observed was measured to be approximately 120 feet long (North-South axis), by approximately 45 feet wide (East-West axis). Area was observed to be covered in shot rock, ranging in size from less than an inch in diameter, up to approximately 2.5 feet in diameter. The shot rock was observed to consist mostly of partially weathered quartzite, with some light brown slate (meta-mudstone) mixed in along the south end. Quartz veins were not observed, however, small pieces of quartz were observed sporadically throughout the shot rock (2-5 inches in diameter). One Joint set was observed within the exposed quartzite, with the measured strike and dip as follows: Strike = N 28 W, Dip = 83 E; Strike = N 39 W, Dip = 84 E. The landfill is owned and operated by Waste Management (WM). The landfill development is being implemented in phases as new solid waste cells are needed. To date, Cell Nos. 1A, 1B, 2, and 3A have been constructed. Cell No’s. 3B, 4 and 5 are currently under construction and are nearing completion. Subgrade preparation activities have been completed in the eastern portion of Cells 3B, 4, and 5 and liner installation is ready to proceed at the approval of this geologic observation report. Rock blasting was performed along the southern, west-central, and northern portions of the cell construction area. The approximate blast area occupies <20% of the 12.7-acre Cells 3B, 4, and 5 footprint. A North Carolina licensed BLE geologist (either Mr. Benjamin Nisbeth or Mr. T.J. Daniel) was on call to be on site within 48 hours post-blasting event to inspect the geologic subgrade. Three (3) geologic observations were performed in the eastern portion of Cells 3B, 4, and 5 on April 14, April 22, and May 14, 2020. Geologic Observation Report: Eastern Portion of Cells 3B, 4, and 5 May 28, 2020 Great Oak MSW Landfill – Randolph Co., North Carolina BLE Project No. J20-13334-02 2 of 4 2.0 GEOLOGIC OBSERVATIONS Pursuant to the facilities Permit to Construct issued January 19, 2016 and in accordance with the facilities blasting plan for Cells 3B, 4, and 5 approved on March 23, 2020 (Appendix A) a brief summary of the geologic conditions encountered in the eastern portion of Cells 3B, 4, and 5 are as follows: • Contractor excavation exposed “unrippable” rock above the proposed subgrade elevation and resulted in the blasting and over excavation of rock from <20% of the 12.7-acre cell area. • In areas where rock had not been exposed, soil was observed as a brown, fine to medium sandy, SILT and is consistent with regolith derives from the Uwharrie Formation and Carolina Slate Belt. • The bedrock encountered within the cell area predominately consisted of a grey-green quartzite ranging in size from three (3) inches to twenty-five (25) feet in diameter. The quartzite was overlain in areas by a tan to white slate (meta-mudstone) and phyllite ranging in size from three (3) inches to two and a half (2.5) feet in diameter. No in situ quartz veins were observed within the exposed rock; however, there were several boulders ranging from three (3) inches to two (2) feet in diameter amongst the shot rock, indicating isolated intrusive quartz veins were present. • Strike and dip values were taken when encountered on exposed in situ bedrock (see Appendix B) and were consistent with the range of values denoted in the Site Hydrogeologic Report (SHR) prepared by Golder Associates and dated December, 2014. A more detailed summary of geologic observations and field sketches for this subgrade inspection of the eastern portion of Cells 3B, 4, and 5 is included in Appendix B of this report. In areas where a greater than four (4) foot separation between the top of rock and proposed landfill subgrade was achieved, as much shot rock as feasible was removed by the contractor. In the event that any shot rock was left behind structural fill was placed above and around these areas and proof rolled with a loaded CAT 740 dump truck to ensure that proper stability was achieved. Geologic Observation Report: Eastern Portion of Cells 3B, 4, and 5 May 28, 2020 Great Oak MSW Landfill – Randolph Co., North Carolina BLE Project No. J20-13334-02 3 of 4 3.0 VERTICAL SEPARATION CERTIFICATION Pursuant to rule 15A NCAC 13B.1624(b)(4), for municipal solid waste landfills, and the facilities approved Permit to Construct, the top of the landfill subgrade and subsequent bottom of the liner system must maintain a minimum four (4) foot vertical separation from the top of bedrock. Prior to blasting within the cell footprint, Survey Carolina, PLLC., was contracted to survey the initial elevations of bedrock. These same northings and easting were surveyed again after blasting was complete and the appropriate amount of shot rock was removed. Table 1 shows the total separation between top of rock and top of subgrade in the eastern portion of Cells 3B, 4, and 5 to ensure that a minimum of four (4) feet of separation was achieved. A plot of the bedrock survey locations in the eastern cell area can be found on Figure 2. A North Carolina licensed geologist was mobilized to the site three (3) times in order to observe the exposed geologic subgrade in the eastern portion of Cells 3B, 4, and 5 and to ensure that no impacts to the integrity of the environmental monitoring system had occurred. All surveyed locations were shown to have a minimum of at least four (4.0) feet of separation between the top of bedrock and the top of subgrade. A separate report will be prepared and submitted for the western portion of Cells 3B, 4, and 5 at a later date. N/A Upon arrival, rep conducted a geologic observation of the exposed shot rock located near grids 12-13. Area of shot rock observed was measured to be approximately 120 feet long (North-South axis), by approximately 45 feet wide (East-West axis). Area was observed to be covered in shot rock, ranging in size from less than an inch in diameter, up to approximately 2.5 feet in diameter. The shot rock was observed to consist mostly of partially weathered quartzite, with some light brown slate (meta-mudstone) mixed in along the south end. Quartz veins were not observed, however, small pieces of quartz were observed sporadically throughout the shot rock (2-5 inches in diameter). The landfill is owned and operated by Waste Management (WM). The landfill development is being implemented in phases as new solid waste cells are needed. To date, Cell Nos. 1A, 1B, 2, and 3A have been constructed. Cell No’s. 3B, 4 and 5 are currently under construction and are nearing completion. Subgrade preparation activities have been completed in the western portion of Cells 3B, 4, and 5 and liner installation is ready to proceed at the approval of this geologic observation report. Rock blasting was performed along the southwestern, west-central, and northwestern portions of the cell construction area. The approximate blast area occupies <20% of the 12.7-acre Cells 3B, 4, and 5 footprint. A North Carolina licensed BLE geologist (Mr. Benjamin Nisbeth) was on call to be on site within 48 hours post-blasting event to inspect the geologic subgrade. Two (2) geologic observations were performed in the western portion of Cells 3B, 4, and 5 on May 4, 2020 and May 7, 2020. The quartzite was overlain in areas by a tan to white slate (meta-mudstone) and phyllite ranging in size from three (3) inches to two and a half (2.5) feet in diameter. Procedures 1. Holes will be laid on a 6’x6’ box pattern to an 8’x8’ box pattern depending on rock density and depth. The average and most used pattern will be the 7’x7’ box pattern. 2. Holes will be drilled 3.5” inches in diameter maximum with an average depth of 15’ deep. So far, the cuts have ranged from 4.5’ to 18’ deep. 3. Holes will be drilled with Tamrock Ranger 800’ or a Sandvik DX800’ (same machine different name depending on year model). Each one of the machines has digital level on it for straight hole setup. 4. All explosives will be brought in daily and no explosives will be stored onsite overnight. Either Elite Blasting or Carolina Drilling will haul all explosives in accordance with the NCDOT and USDOT. 5. Dry holes will be loaded with a 1 ¼”x8” stick of Riodin dynamite and ANFO. Wet holes will be loaded with a booster and Riohit emulsion sticks. All holes will have a minimum stemming 5.5’. 6. Only non-electric blasting caps will be used 25/500ms factory delayed. 7. SDS sheet will be attached. 8. All holes will be loaded with non-sparking tools (depth gauges and loading poles). 9. The rock in this area is of medium thickness and will only take 3’ to 11’ of the boreholes to loaded with explosives. In this case our average stemming (78m) will be held at 5.5’ to 9’. 10. All holes will be wired with a minimum of 8ms of delay between them giving us 1 hole per delay. 11. The holes then will be walked over to make sure all connections are clean. 12. Vibratech will have all seismographs in place before any blasting is done. 13. Once the area is cleared by Earnhardt Grading and Carolina Drilling a series of horns will be sound. -Three short blast for a 5 minute warning -Two long blast for a 1 minute warning -One long blast for an all clear, back to work after the shot has been inspected to assure all holes have fired. Drilling Equipment -A Sandvik DX800 or Tamrock Ranger 800 will be used, this machine has a digital level on the boom that reads in the cab. Seismology and Preblast Surveys Vibratech is handling the preblast surveys and seismology on this project. All machines shall be calibrated within 6 months of the blast. Post Blast Report or Shot Report A shot report used by Carolina Drilling blaster will be attached to this plane. It will show the following as required. 1. Rep observed weathered and partially weathered light brown to white phylite in the south 150 feet, with gray quartzite in the middle section (approximately 85 feet in length), and weathered phylite with pieces of quartzite in the north 65 feet. Rep did not observe any exposed quartz veins today.
