PTO Issued January 21, 2016. Expires January 25, 2021. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - Permit to Operate was issued on 01/21/2016 and is set to expire on 01/25/2021. Pursuant to Rule 15A NCAC 13B .0201 (g) no later than 07/25/2020, the permittee must submit a request to the Section for a permit review and must update pertinent facility plans including but not limited to facility operations and waste screening plans in accordance with Rule 15A NCAC 13 B .0535. 02 - This facility has required signage at the entrance and directional signs inside of the landfill footprint. 03 - Roads are constructed of all-weather materials and are being maintained well. Facility has locking gate and fencing for security. 04 - This facility is permitted to receive waste generated in Wilson County. 05 - Edge of waste markers present. 06 - Asbestos area is marked and is being manage well. 07 - Monitoring wells are locked and tagged. 08 - Facility has adequate soil cover pile on-site for use. 09 - Cover logs were available and reviewed. 10 - Leachate inspections and asbestos entries are written on the cover logs. 11 - Working face of this facility is kept small, and cover material is stored on the working face for quick access. 12 - This facility has 6 certified Landfill Operations Specialist: Tammy Webb, expires 11/15/2020, Paige R. Aycock, expires 10/19/2022, Kenny E. Matthews, expires 10/19/2022, Melinda R. Wall, expires 10/19/22, Jonathon Mark Denton, expires 11/04/2022 and Robert B. Patterson, expires 04/11/2023. Also, the facility has 1 Certified Technical Associate of Landfill Operations, George Andrew Davis, expires 05/06/2020. 13 - Land Clearing and Inert Debris area is well maintained and is being covered monthly at a minimum. Site is less than 2 acres. 14 -There is a Convenience site located inside of the landfill footprint. Containers are labeled with the type waste received. This site is managed well. 15 - All MSW waste from this Convenience Center goes to Waste Industries, Black Creek Road Transfer Station, permit # 9808T in Wilson, NC. 16 - There is a Recycle/Electronics Center located in the landfill footprint. This center is managed well. 17 - There is a Wood Processing and Yard Trash Composting Area located in the landfill footprint. This site is managed well. Temperature records are being kept for Pathogen reduction and Vector attraction reduction. 18 - It was noted that some piles had not been recorded at 131 degrees or higher for 3 consecutive days. This MUST be met (documented) to ensure the pathogen reduction has been met if distributed to the public. 19 - The wood processing area has adequate fire lanes in the event of a fire. 20 - Load inspections and Screening records were reviewed during this comprehensive facility compliance inspection and were up to date. 21 - Incoming load tickets were reviewed. Need to Identify (Name, Address, Vehicle Tag Number) cash customers on load tickets in the event of a hot load or hazardous waste or any other types of non-conforming waste is found it can be traced back to the generator. 22 - All White Goods and scrap metal are stored in open top containers. White Goods containing CFC’s are placed into the open top containers upright for Foss Recycling to remove the CFS at their facility. A yearly record is provided to this facility from Foss Recycling on the CFC removal. 23 - Landfill Gas Monitoring Quarterly Reports for 2019 available and reviewed. 24 - Ground Water Semi Annual Monitoring Well Reports for 2019 were available and reviewed. 25 - Digital pictures were taken during this partial facility compliance inspection. Some are attached to this report. 26 - Overall, this facility is being managed well.
