Site closed in accordance with the Closure letter dated December 29, 1995, and NC Solid Waste Management Rule 15A NCAC 13B .0510. PURPOSE OF SITE VISIT: Comprehensive Facility Inspection. STATUS OF PAST NOTED VIOLATIONS: 1. 15A NCAC 13B .0502(2)(a)(ii) — Corrective actions are ongoing for landfill gas (LFG) exceedances at the facility boundary. As required, the facility has continued to submit Operations, Monitoring, and Maintenance Reports to the Section for the Landfill Gas Collection and Control System (LFGCCS) and the facility perimeter monitoring probes. Per the July 2019 through December 2019 Operations, Monitoring, and Maintenance Report (FID 1386876), repairs to the system conducted in August 2019 included repair of the drain line from the flare station to the condensate tank and repair of laterals connecting select monitoring probes to the wellfield. Please continue to work with Ms. Jaclynne Drummond, Section Hydrogeologist, regarding ongoing LFG corrective actions at the facility. OBSERVED VIOLATIONS: 1. 15A NCAC 02C .0108(p) states: "Each non -water supply well shall have permanently affixed an identification plate. The identification plate shall be constructed of a durable, waterproof, or rustproof material and shall contain the following information: Page 1 of 6 NORTH CAROLI D_EQ�� UeVnNnenl of Enrlrwmrenbl Ouel� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section (1) well contractor's name and certification number; (2) the date the well was completed; (3) the total depth of the well; (4) a warning that the well is not for water supply and that the groundwater may contain hazardous materials; (5) the depth to the top and bottom of each screen; and (6) the well identification number or name assigned by the well owner." The site -wide Groundwater monitoring network includes three monitoring well locations that serve to monitor the closed MSW landfill facility. Additionally, the site -wide LFG monitoring network includes thirteen monitoring well locations that serve to monitor the closed MSW landfill facility. As discussed post -inspection with Mr. Eric Foster, Avery County Solid Waste Director, it was observed that some identification plates were maintained on environmental monitoring wells at the facility, but identification plates were not maintained on all groundwater and LFG monitoring wells at the site (photos #1, #2, #3, & #4). See Tables #1 & #2 below for additional information regarding monitoring well status. Post -inspection, Avery County stated the facility had informed the site's Project Engineer of the necessity to gain compliance with 15A NCAC 02C .0108(p). To achieve compliance, Avery County Solid Waste Management Facility Permit No. 06-01 must affix a permanent identification plate on all facility monitoring wells as specified in 15A NCAC 02C .0108(p). The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. On -site February 25, 2020, to conduct comprehensive facility inspection of the closed MSW landfill. Post inspection, additional facility research conducted which concluded on March 5, 2020. 3. Access to the site is located off Brushy Creek Road (S.R. 1101) and is controlled by a lockable gate. 4. Access roads at the site were maintained. Crushed asphalt appeared to have been recently applied on some areas of site access roads. As discussed post -inspection, continue to maintain site access roads as necessary. 5. Edge -of -waste (EOW) markers were observed around the closed MSW landfill and appeared to be maintained. Continue to maintain EOW markers at the facility as necessary. 6. No apparent leachate breakouts or seeps were observed during this inspection. 7. Semi-annual groundwater and surface water monitoring reports for the facility dated January 2, 2020 (for Fall 2019 sampling event) and June 28, 2019 (for Spring 2019 sampling event) were available for review. As required, continue to monitor all groundwater and surface water sampling locations at the facility and submit reports to the Section in a timely manner. 8. The groundwater and surface water monitoring network for the closed MSW landfill is comprised of three groundwater monitoring wells (MW-1, MW-2, and MW-3R), and two surface water monitoring locations (SW-1 and SW-2). During this inspection, all groundwater and surface water monitoring locations at the facility were observed. Continue to maintain access for all environmental monitoring locations at the facility. (Note — see Table #1 of this inspection report for additional information on groundwater monitoring wells at the facility.) 9. The facility's LFGCCS includes a Well Extraction Field with ten extraction points (designated #W-1 to #W-10), a Flare Station, and Condensate Tank. The facility's LFG Monitoring Well (Perimeter) Monitoring Program includes thirteen monitoring well locations (#P-1 to #P-13) around the perimeter of the landfill (located on and off -site), and six airport structures (located on the adjacent Avery County Airport Authority property west of the facility). During this inspection, LFG monitoring locations #P1 through #P-12 were observed. Access to LFG monitoring locations was maintained. (Note — see Table #2 of this inspection report for additional information on LFG monitoring wells at the facility.) 10. During this inspection, ground cover appeared to be well established on the slopes and cap of the closed MSW landfill (photos #5 & #6). As discussed post -inspection, continue to maintain ground cover on the areas where LFG maintenance activity was recently conducted at the facility, including the areas around extraction well vacuum risers #W1, #W3, #W9, and #W10 (photos #7 & #8). After mowing events occur, continue to ensure the landfill cap and side slopes are inspected for subsidence, cracking, signs of erosion or damage, and that repairs are conducted as necessary. l�l 6. Continue to maintain all erosion control measures at the facility as necessary including rip -rap channels, sediment detention basins, permanent diversions, and inlet/outlet stabilization structures.
