Box 729 Burnsville, NC 28714 PARTICIPANTS: Kris Riddle, Environmental Senior Specialist, Solid Waste Section (Section) STATUS OF PERMIT: The Small Type I Facility operated under Yard Waste Notification (YWN) #100-002. The Section received a Notice of Ceased Operation of a Facility form for YWN-100-002, dated October 18, 2019. PURPOSE OF SITE VISIT: Closure Inspection. STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. On -site December 5, 2019, to determine YWN-100-002 closure status. The Section received a Notice of Ceased Operation of a Facility form for the site, dated October 18, 2019. The facility was permitted to receive land clearing debris: including stumps, whole trees, tree limbs, branches, untreated wood; and yard trash: including grass, leaves, and brush/shrubs. 3. During this inspection, stumps and woody debris were observed on -site (photo #1). No unpermitted waste was observed on -site during this inspection. 4. Post -inspection, on December 30, 2019, the Section contacted Mr. Tim Parker, site owner/operator, to discuss facility operations and the closure inspection. As discussed with Mr. Parker post -inspection, the facility requested to rescind the closure request and submit a YWN Facility Renewal Form. An email, which included a YWN Facility Renewal Form digital link, was sent to the facility on December 30, 2019. 5. Additionally, the site was previously approved as temporary disaster debris site #DS100-01. In addition to the YWN closure request, the facility requested temporary disaster debris site #DS 100-01 be removed from the Section's temporary disaster debris site inventory. Post -inspection, DS100-01 was removed from the Section's temporary disaster debris site inventory. 6. All photographs taken by Kris Riddle on December 5, 2019. I.
