with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. All photos included in this report were taken by Susan Heim during the facility inspection on 10/10/2019.
2. The facility is permitted to receive, consolidate and transfer yard trash (as defined in 15A NCAC 13B
.0101(55)) for storage and removal. The site receives and manages leaves collected curbside by the City of
Winston-Salem Sanitation Division.
3. Operations at the site are managed by Wallace Farms, Inc., a contractor that also manages the city's Overdale
Road Yard Waste Facility (permit # 34-09) and Forum 52 Large Type 1 Compost Facility (permit # 34-23).
4. City trucks unload leaves at the site and push them into piles. Wallace Farms staff then manage the leaf
stockpiles during their temporary storage at the site, and load them into trucks, transporting them as needed.
5. The facility had not yet opened for the season. Mr. Mack stated that leaf collection begins November 4, 2019.
6. During this inspection, preparations were being made to open the site. Mr. Mack advised that gravel will be
brought onto the site to fill any low areas that could collect water, and that the white line that indicates the 50'
buffer requirement will be repainted so that it is clearly visible during operations.
7. The facility is secured by a chain link fence that encloses the property, and unauthorized access is prevented by
two locking gates that are kept locked except when facility staff or Wallace Farms staff are present.
8. The site was observed to be clean and well -maintained.
9. Proper signage was observed at the entry gate of the facility.
10. The site is sloped so that surface water is directed to the city's storm drains.
11. The entire site is asphalt paved. Near the center of the site, a portion of the asphalt has crumbled from heavy
equipment use. This is the area where gravel is added as needed to maintain a level surface that prevents
ponding water and allows equipment to maneuver safely.
12. At the time of this inspection, driver training was in progress, and several city trucks with leaf collection
equipment were being maneuvered in training courses by city staff who would be operating the equipment once
curbside leaf collection begins. Mr. Mack explained that training takes place annually so that all drivers become
refamiliarized with the vehicles and equipment that they operate for only a few months each year.
13. A metal loading ramp was located in the northwest corner of the site. Prior to the start of the operating season,
the ramp will be staged near the center of the site so that it can be used by loaders to fill trailers that will
transport the leaf mulch offsite for composting. No composting operations take place at the facility.
14. The facility annual report for FY 2018-19 was reviewed, and indicated that 13,627.80 tons of leaves were
received at the site during the last operating season. This tonnage is significantly less than that of FY 2017-18,
when the facility received a total of 22,005.84 tons of leaves. A review of the facility history indicates that
previous operating seasons have generated between 10,000 and 20,000 tons of leaves per year.
15. Please ensure that an individual with operational responsibilities is onsite during all operations at the
facility, as required by the Permit to Operate.
