This permit is in the names of: Currituck County (owner) and Republic Services of North Carolina, LLC (operator). PURPOSE OF SITE VISIT: Follow up Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - This follow up facility compliance inspection was conducted by John College, NCDEQ-DWM to ensure compliance is being met due to the June 18, 2019 observed violations. 02 - No windblown trash was noted during this inspection around the transfer station. 03 - Tipping floor and loading bay were clean as they can be during working hours. 04 - One small leak was noted on a stored trailer and trailer was tarped. A 5 gallon bucket had been placed under the small leak. 05 - Screening records were up to date and indicates non-conforming waste pulled from the waste stream to be taken to approved areas. 06 - Reviewed incoming load tickets. 07 - Fire suppression equipment is available with several charged fire extinguishers and water hoses. 08 - Certified Transfer Station Operation Specialist: David Lowe, expires 5/25/2022, Charlie Rome, expires 3/12/2022 and James Bolen, expires 10/9/2020. 09 - No evidence of Scavenging of waste at the transfer station during this inspection. 10 - The white goods area was also inspected and found that all white goods containing CFC’s were upright until the CFC has been removed. These items are marked with a red X and a small label after the CFC is removed and then they are place on the scrap metal pile for removal and sale. 11 - The yard waste area is in compliance. 12 - Scrap tires are stored in enclosed box containers. Some are in need of removal from the transfer station to the containers. 14 -This facility is now keeping a daily checklist that was developed by Republic Services to be used and signed by Bowen staff. This record was reviewed and is also sent to Republic Services on a weekly schedule. 15 - LP tanks are in need of removal from the transfer station. Some still may contain fuel. 16 - Overall management of this facility has improved. 17 - Digital pictures were taken during this inspection.
